Same day pay available POSITION PURPOSE: Organizes and directs assigned unit duties to provide quality nursing care to residents. ESSENTIAL FUNCTIONS OF POSITION: Responsible during the shift for the total nursing care of residents in the assigned unit. Assumes responsibility for compliance with Federal, State, Local, and company regulations. Explains procedures and treatments and care plan to resident. Administers prescribed medications and treatments in accordance with approved nursing techniques. Transcribes physician's orders. Charts progress notes in an informative, factual manner that reflects the care administered as well as the resident's response to care. Receives /gives report from/to nurse he /she is relieving/coming on and records sufficient information and implements appropriate follow-up action as necessary Follows established procedures for charting and reporting all reports of incident/accidents for res idents and employees. Conducts frequent rounds to evaluate effectiveness of care delivery. Prepares equipment and a ids Physician during treatment and examination of resident. Maintains awareness of comfort and safety needs of resident. Observes resident, records significant conditions and reactions, and notifies supervisor or Physician of resident's condition and reaction to drugs, treatments, and significant incidents . Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of resident. Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. Assists in completion of the care plans (acute and long term) and is involved in the care conference as deemed necessary. Participates, reports to DON/ Administrator and conducts (as appropriate) investigations related to any alleged abuse. Ensures that documentation is reflective of the care plan and that the approaches on the care plan are being followed. Assures that assigned CNA's are aware of the resident care plan, and ensure that the nursing assistant refers to the care plan prior to administering care. Monitors that CNA's documentation is completed daily . Takes report form CNA's to facilitate accurate documentation in resident medical record. Follows company policy and procedure in the event a restraint must be utilized. Coordinates residents' rehabilitation program with therapists to ensure 24 hour rehabilitation needs are met. Identifies residents at risk for nutritional problems and implements preventative measures. Records new/changed diet orders. Forwards information to the Dietary Department. Makes beds, bathes and feeds resident, when necessary. Assures documentation is accurately completed for admission, discharge and transfer. Supervises and evaluates implementation of the "Resident's Rights". Participates in department or unit quality improvement activities. Follows facility procedure requesting supplies and charges for items ordered through Central Supply. Participates and attends periodic in-services and staff meetings. Perform additional duties as assigned. EDUCATION/ REQUIREMENTS: Current State of LVN/LPN with a minimum of 2 years related experience; or equivalent combination of education and experience. Good standing in the state registry is required.

Responsibilities • Performs basic nursing care and patient procedures as required. • Participates in conferences and serves on committees as requested. • Observes, reports, and documents changes in patient condition, observations made, complaints voiced, and nursing care performed. • Participates in patient and family health education as necessary. • Provides a safe and secure environment for patients. • Prepares for and assists with diagnostic and therapeutic treatments. • May administer I.V.F. in accordance with the State Nurse Practice Act. • Checks, irrigates, and maintains drainage tubes as needed. • May insert N/G tubes after training (KY & VA). • May administer tube feedings. • Suctions patients according to hospital policy and procedure. • Functions in emergencies and on the code team as designated. • Performs catheterizations and provides catheter care. • Administers oral, intramuscular, intradermal, subcutaneous, I.V., P.B., and approved medications. • Administers I.V.P. medications as approved (Virginia only). • Cares for patients receiving oxygen therapy and analyzes oxygen concentration of O2 tents, isolettes, and croupettes. • Administers respiratory treatments (I.P.P.B.). • Applies sterile and unsterile compresses and bandages of all types and styles. • Answers call lights and assists patients with personal hygiene and activities of daily living. • Tests urine for sugar, acetone, and pH, weighs patients, and obtains vital signs. • Cares for patients in isolation. • Assists with physical and vaginal examinations and obtaining Pap smears. • Administers post-operative care, including coughing, deep breathing, turning patients, and obtaining vital signs. • Prepares patients for surgery, including completion of pre-operative checklists. • Applies ace bandages, all types of traction, hot water bottles, and ice caps. • Measures and records intake and output. • Collects specimens, including blood, urine, stool, and sputum. • Administers post-mortem care and transportation. • Administers enemas, including retention, cleansing, therapeutic, and colonic irrigation. • Maintains cleanliness, safety, and efficiency in nursing care. • Responds appropriately in emergency situations, providing patient care, initiating CPR, and performing duties designated by the RN or physician. • Assists with the care and security of patient valuables. • Checks narcotics and barbiturates, reports discrepancies, completes incident reports, and submits documentation to appropriate management. • Cares for patients with radium and assists physicians with removal of radium. • Prepares used equipment for re-sterilization. • Cleans and maintains storage of equipment and requests repairs as appropriate. • Follows up with physicians and staff regarding equipment performance. • Complies with hospital policies for obtaining, documenting, and following through on telephone and verbal orders. • Monitors glucose using a glucose monitoring device. • Collects Quality Improvement data as requested. • Uses universal precautions. • Performs other related duties as assigned. Qualifications Education • Graduate of an accredited school of practical nursing. Licenses & Certifications • Must maintain current licensure in the state of practice. • Must successfully complete the I.V. Therapy Course. Experience • Six (6) months to one (1) year of on-the-job training is required to attain proficiency in the performance of assigned duties. Licenses & Certifications • Registered Nurse required. • BLS certification required. • Completion of Intermediate Fetal Monitoring Course (AWHONN) within six (6) months of hire required. • Neonatal Resuscitation Program (NRP) certification required. • De-Escalation Training required within six (6) months of hire.
