Same day pay available POSITION PURPOSE: Organizes and directs assigned unit duties to provide quality nursing care to residents. ESSENTIAL FUNCTIONS OF POSITION: Responsible during the shift for the total nursing care of residents in the assigned unit. Assumes responsibility for compliance with Federal, State, Local, and company regulations. Explains procedures and treatments and care plan to resident. Administers prescribed medications and treatments in accordance with approved nursing techniques. Transcribes physician's orders. Charts progress notes in an informative, factual manner that reflects the care administered as well as the resident's response to care. Receives /gives report from/to nurse he /she is relieving/coming on and records sufficient information and implements appropriate follow-up action as necessary Follows established procedures for charting and reporting all reports of incident/accidents for res idents and employees. Conducts frequent rounds to evaluate effectiveness of care delivery. Prepares equipment and a ids Physician during treatment and examination of resident. Maintains awareness of comfort and safety needs of resident. Observes resident, records significant conditions and reactions, and notifies supervisor or Physician of resident's condition and reaction to drugs, treatments, and significant incidents . Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of resident. Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. Assists in completion of the care plans (acute and long term) and is involved in the care conference as deemed necessary. Participates, reports to DON/ Administrator and conducts (as appropriate) investigations related to any alleged abuse. Ensures that documentation is reflective of the care plan and that the approaches on the care plan are being followed. Assures that assigned CNA's are aware of the resident care plan, and ensure that the nursing assistant refers to the care plan prior to administering care. Monitors that CNA's documentation is completed daily . Takes report form CNA's to facilitate accurate documentation in resident medical record. Follows company policy and procedure in the event a restraint must be utilized. Coordinates residents' rehabilitation program with therapists to ensure 24 hour rehabilitation needs are met. Identifies residents at risk for nutritional problems and implements preventative measures. Records new/changed diet orders. Forwards information to the Dietary Department. Makes beds, bathes and feeds resident, when necessary. Assures documentation is accurately completed for admission, discharge and transfer. Supervises and evaluates implementation of the "Resident's Rights". Participates in department or unit quality improvement activities. Follows facility procedure requesting supplies and charges for items ordered through Central Supply. Participates and attends periodic in-services and staff meetings. Perform additional duties as assigned. EDUCATION/ REQUIREMENTS: Current State of LVN/LPN with a minimum of 2 years related experience; or equivalent combination of education and experience. Good standing in the state registry is required. Be in charge of a group of dedicated nurses, ensuring they are able to provide the highest quality of care for residents. Overview The Registered Nurse requires substantial specialized knowledge, judgement and nursing skills based upon the principles of psychological, biological, physical, and social sciences in the application of the nursing process in the respective departments in which assigned. The RN must collaborate with physicians and multidisciplinary team members; provide physical and psychological support to patients, friends, and families; supervising assigned team members and students. Responsibilities • Completes patient assessments, formulates plans of care, oversees implementation of plans of care, and evaluates and adjusts plans of care utilizing critical thinking skills. • Performs other nursing acts authorized or limited by the Board and consistent with American Nurses Association Standards of Practice or Standards of Practice established by nationally accepted organizations of registered nurses. • Provides care, counseling, health teaching, and health promotion utilizing Teach Back methodology as affirmation of comprehension. • Administers medications, intravenous therapy, and treatments as prescribed by a Licensed Independent Practitioner (LIP). • Supervises, delegates, and follows through with other personnel in the performance of activities relating to nursing care. • Mentors and precepts students and orients new staff as assigned. • Reports pertinent information through the chain of command to the Clinical Nurse Manager, Nurse Manager, or House Nurse Coordinator. • Participates on various teams, committees, and performance improvement initiatives. • Ensures a safe and secure environment for patients, coworkers, and self. • Participates in Discharge Planning and Transition of Care activities and is able to define patient needs. • Serves as a patient advocate and escalates patient-related concerns in a timely manner. • Documents pertinent findings in the medical record in a timely manner. • Assists patients in performing activities of daily living and functions as an active team member. • Assumes responsibility for personal professional development. • Utilizes effective communication skills and active listening and exhibits a compassionate and empathetic attitude. • Practices Hands Off communication utilizing SBAR and performs Bedside Shift Reporting utilizing SBAR unless contraindicated. • Functions as a positive influence and role model and adheres to the professional code of conduct. • Respects the dignity and confidentiality of all matters relating to patients. • Refers to Special Addendum and Staff Nurse Position Description. • Performs other job duties as assigned. Emergency Room Training Requirements • State and federal regulations related to emergency services. • Nursing management of trauma. • Nursing management of cardiac-related disorders. • Performance of EKG procedures, EKG interpretation, and appropriate nursing interventions. • Pharmacological intervention including IV therapy. • Patient assessment and triage. • Nursing management of pediatric emergencies. • Medicolegal aspects, particularly related to proper documentation. • Nursing management of obstetrical emergencies. • Patient and family emotional responses. • Cardiopulmonary resuscitation and airway management. Qualifications Education • Associate's Degree required. • Graduation from an accredited School of Nursing required. • Bachelor's Degree preferred. Experience • Up to one (1) year of experience preferred. • Minimum of one (1) year acute care nursing experience preferred. • One (1) year of experience should be relevant to the specific department in which the nurse will be working. • Refer to addendums for additional department-specific requirements. • Experience with Electronic Health Records (EHR) preferred. Labor & Delivery Training Requirements • Admission of labor patients. • Care of labor patients. • Delivery of labor patients. • Recognition and nursing care of abnormal conditions. • Fetal monitoring and nursing interventions. • Appropriate nursing interventions. • Management of obstetrical emergencies. • The nurse shall be adequately oriented to unit policies, procedures, and equipment. • Orientation shall include safety procedures, infection control practices, and performance improvement activities. Qualifications Education • Associate's Degree in Nursing required. • Bachelor's Degree in Nursing preferred. Experience • One (1) to three (3) years of experience preferred. Required Skills, Knowledge & Abilities • Performs initial assessment to include physical examination of OB patients, recognizes abnormal signs and symptoms, and takes action as indicated. • Performs special OB procedures as required. • Sets up, operates, and cleans OB equipment. • Provides routine post-partum care including ongoing assessment.
