The Heart and Vascular Center at UChicago Medicine is at the forefront of cardiac surgery, specializing in diagnosis, treatment and prevention of heart conditions and heart disease. In this role, you will utilize advanced assessment skills and knowledge of clinical therapeutics to evaluate, diagnose and manage critically ill patients. The APP orders and interprets appropriate diagnostic tests, therapeutic interventions and prescribes pharmacological agents as necessary. As an APP, you will maintain continuity of patient care through patient follow-up, monitoring of patient progress and modifying plan of care as necessary. The APP applies knowledge of new technology and resources demonstrating advanced or specialized expertise in caring for a variety of cardiac surgery patients including those with complex valve disease, coronary artery disease, disorders of the aorta, advanced heart failure requiring heart transplant or left ventricular assist device placement. The Family Connects model is an evidence-based and successfully demonstrated program that connects parents of newborns to the community resources they need through postpartum nurse home visits. The model was developed in 2008 through a partnership with Duke University, the Center for Child & Family Health (CCFH), and the Durham County Health Department. As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents’ needs, and helps to identify community services or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally. A Family Connects Home Visiting Nurse provides home visiting to all families with newborns in Chicago. The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The Home Visiting Nurse will work in a team environment of clinical and non-clinical staff in order to schedule visits, and support families in the community using the evidence-based Family Connects home visiting protocol. Initiate and coordinate patient education: Ensure that patients understand educational materials and can exhibit return demonstration when pertinent. Provide teaching to staff on both a formal and informal basis. Participate in community outreach education, patient support groups or specialty-related educational programs offered to hospital and community nurses, as requested. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. In this role, the Nurse Associate (RN) is responsible for coordinating diagnostic and therapeutic regimens for a specified group of patients, and maintains cohesiveness between clinical research, physicians, and patients. The Nurse Associate coordinates physician activities with referring physicians, nursing staff, hospital administration, ancillary services, support services, insurance companies and other departments/sections and agencies as needed, and assists the physician with a wide range of activities necessary to manage clinical research programs.
