The home health environment is technology‑driven; therefore, the Licensed Practical Nurse uses email as a primary communication tool and completes all documentation electronically using a laptop‑based medical record system. The patient population represents a broad range of ages, diagnoses, and levels of acuity. The Licensed Practical Nurse practices under the direction of a Clinical Team Manager. Employment begins with a comprehensive 6–10-week orientation alongside an experienced preceptor to ensure clinical readiness and confidence before independent practice. Additional responsibilities may include participation in performance improvement initiatives, policy and procedure review and development, and engagement in research activities. Education Completion of an accredited school of nursing program is required. Experience Must meet all requirements of an LPN I ladder to advance to LPN II. QUALIFICATIONS: Current Wisconsin LPN Licensure American Heart Association Healthcare Provider Basic Life Support (BLS). PHYSICAL DEMANDS: Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties WORK ENVIRONMENT: Frequent exposure to sharp objects and instruments.
