MAJOR AREAS OF RESPONSIBILITY: Provides skilled nursing care in a patient’s home as ordered by the attending physician. Performs care by following order sets and goals made by RN. Observes and monitors patient conditions. Notifies RNCM of any changes in patient condition. Administers medication as prescribed by the physician. Counsels patient and family on the disease/injury processes and how to manage. Incorporates patient and family in development of plan of care. Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions. Listens to patient and family members. Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines. Documents all aspects of treatment, assessments, and patient education. Maintains active LPN license in state of employment. Communicates with scheduler any changes outside the normal. Participates in all on-call requirements and case conferences. Attends in-service trainings and mandatory agency meetings. Maintains a professional appearance as a representative of the company. Understands and practices agency policies and procedures. High school diploma or equivalent. Completed training program approved by the medical director and governing body (as required by CMS guidelines) prior to operating the water treatment system. BONENT or CHT certification preferred (or as required by state regulation). Minimum of one year's experience in a hospital or clinical setting preferred. Basic computer skills and proficiency in MS Word and Outlook (functional proficiency in DaVita's computer systems required within 90 days of hire). Successful completion of a pre-employment color blind test (accommodations may be provided).
