ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Measures and records patient vital signs; records patient interview and medical history. Catalogs and communicates patient needs to the PCP, Clinician, Specialist and/or other clinical personnel. Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians' instructions to patient. Observes patients and reports any changes in patient conditions to the PCP, Clinician, Specialist and/or other clinical personnel. Summary GENERAL SUMMARY The Specialty Licensed Practical Nurse performs a wide variety of patient care activities and accommodative services to support a defined patient population program or service. Reports to Practice Manager/Supervisor and works in close cooperation with and under the clinical guidance of the practice physician(s). ESSENTIAL DUTIES AND RESPONSIBILITIES Demonstrates excellence in nursing practice characterized by quality patient care and participation in formal and informal patient and staff education. Provides developmentally appropriate psychosocial and clinical care. Performs other duties and tasks as assigned. LICENSES AND/OR CERTIFICATIONS Current State license as a Licensed Practical Nurse. MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS Professional knowledge of nursing theory and practice. Technical and professional knowledge and skills unique to the business and/or clinical services provided, usually acquired through 3-5 years’ progressive clinical experience in the related specialty preferred. Pediatric experience preferred. CPR Training required and/or must be obtained within 45 days of hire. Must provide record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing. WORKING CONDITIONS Expected to work in a patient care environment with minimal exposure to environmental hazards such as, but not limited to, excessive noise, dust, or extremes in temperatures. Exposure to communicable diseases and moderately adverse working conditions due to the need to perform certain patient care activities can be expected. Hampton, Virginia Overview Performs duties under the direction or supervision of a licensed medical practitioner or a registered nurse. Responsible for conducting a focused assessment of the patient to include vital signs and gathering data, implementing established plans of care, and evaluating effectiveness of treatments in the medical practice setting. May float to other practices within the region providing operational support as needed. What you will do Accurately completes all required clinical documentation consistently per Riverside Medical Group standards. Maintains documentation for quality assurance to include but not limited to point of care testing, medication logs and annual wellness visits. Works with providers to assure care gaps are routinely addressed with each patient. Documentation is accurate, legible and completed same day. Knowledgeable and skillful in lab functions and lab proficiency testing. Maintains excellent clinical competency by adhering to the standard clinical expectations for Riverside Medical Group. This includes updating, verifying, and maintaining patient records by properly interviewing the patient and confirming purpose of visit. Prepares patient for examination by obtaining vital signs proficiently (including manual vital signs when mechanical devices are not available). Prepare and sanitize exam rooms before and after patient appointments. Administer medications and vaccines under the supervision of a physician. Assists with processing prescription refill requests for approval and forwarding to pharmacies. Maintains medical inventory of supplies and keep exam rooms stocked appropriately. Keeps any necessary equipment in operating order and facilitating preventive maintenance when needed. Ensures compliance with healthcare regulations with scope of practice, HIPAA, and OSHA standards. Completes all required competencies and education to maintain skill level to provide safe patient care. Provides patient education as needed and educates patients as necessary about the medications prescribed for them. Demonstrates positive patient care by explaining the importance of the completion of registration and patient medical history forms and how it relates to their care at our facility. Provides patient follow up with a sense of urgency regarding lab calls, patient phone calls, prescription fills and refills, referrals and appointment scheduling. Answer telephone calls from patients, pharmacies, physicians, vendors, etc. by close of business. Communicate lab/pathology results and schedules additional tests per provider order. Ensures proper reimbursement from insurance companies by performing pre-authorization for prescription drugs, procedures, and patient tests. Demonstrates the ability to build trusting collaborative relationships with a multi-disciplinary team to support patient care needs.
