Referral Bonus Opportunities Qualifications : Active LPN or RN License issued by NYS Minimum of 1 year experience in a homecare setting. Current BLS Certification Empathy, Reliability , and a commitment to high-quality care What You'll Do : Perform physical and Health Assessments before making critical decisions. Provide health promotion, counseling, and education. Administer medications and other personalized interventions. Coordinate care, in collaboration with a wide array of health care professionals. Document nursing notes regarding patient care daily. Must be able to lift 50lbs. Must be able to stand/work for extended periods Additional responsibilities as required.
