Job Duties Responsibilities: Patient Care Collects data from patient, family or significant others and records on Initial Data Base form. Twenty-four hour nursing assessment and provides data to RN for MDS inclusion. Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse. Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, resident care, TPR) according to established standards. Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed. Documents pertinent information regarding patient's responses and nursing interventions in patient's medical record.
