HOW YOU'LL WORK: You'll provide skilled nursing care in a patient's home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions and notify case manager of any changes in the patient’s condition. Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions. Family Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Operations: Attend in-service training and mandatory agency meetings. HARD & SOFT SKILLS: Compassionate communicator with a positive attitude Patience is a virtue when working with patients, families, physicians, and coworkers Attention to detail is critical, as is being observant and following directions REQUIREMENTS: Licensed Practical Nurse (LPN) with current license in the state of employment Minimum one-year experience as a LPN in an acute care setting. Maintains pharmacy cabinet and recording of patient samples. Completes patient assistance paperwork/requests as needed. Preforms basic triage per specific office protocol procedures. Returns calls as needed. Responds to MyChart as needed. Works in-baskets, daily work flow logs, and work queues in EMR/Practice Management system. Answers incoming calls, schedules appointments, checks-in and out patients, and completes pre-certifications for prescriptions, procedures, and surgeries as needed. Operates EKG, cardiac monitors, and other equipment to administer routine diagnostic tests as needed and other clinical duties such as phlebotomy and cast removal as assigned, (see Appendix A for specifics) as well as injections of controlled substances that only a LPN or RN is licensed to administer. Maintains safe, secure, and healthy work environment by following and enforcing standards and procedures; complying with legal regulations. Understands and adheres to the legal responsibilities and requirements with the medical assistant role.
