Provides effective education to patients that promotes health and wellness. Provides education and information to the practice team that is timely and relevant to the goals of the medical home. Performs pre-visits and patient outreach to assure closure in care gaps. Works quality dashboards to assure care gaps are being addressed. Care documentation is accurate, assuring patient safety.
