Follows up on patient care responsibilities; communicates incomplete assignments and patient needs with other disciplines to assure continuity of care; transcribes orders accurately and performs chart audits of transcription accurately and according to facility guidelines. We are looking for AM & PM 12-hour shift coverage. What does the Nurse do? (including but not limited to) As a Nurse, you are responsible for providing basic patient care under the supervision of a registered nurse, including taking vital signs, administering medications, preparing patients for exams, collecting specimens, assisting with procedures, educating patients on self-care, and documenting patient information thoroughly; all while maintaining clear communication with the healthcare team and patients. Responsibilities Essential duties and responsibilities (including but not limited to) Participates in the planning, coordinating, and documenting of total patient care in conjunction with the multidisciplinary treatment team Accurately reports and records the client’s condition and care, including signs and symptoms which may be indicative of change Accurately monitor skin assessments with proper documentation and reporting of changes Completes, maintains, and documents accurate and relevant clinical notes Ensures timely and accurate clinical documentation for all nursing clinical services provided Participation in education, in-services, and other training and meetings as needed Facilitates appropriate nursing action(s) in response to administrative directives and a changing healthcare environment Ensures all MD orders are followed and documented accordingly during the assigned shift Manages medication administration, monitoring, and proper storage of medication and complies with all medication management policies and procedures Qualifications Graduate of an accredited school of nursing Must have active Licensed Practical Nurse in the state of Louisiana. Minimum of 1 year experience as an LPN, experience in behavioral health care preferred Certification in Basic Life Support Certification in non-violent crisis intervention
