Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs. Assists with the provision of palliative care in accordance with facility, state, and federal regulations. Monitors the effectiveness of interventions. Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients. Collaborates with primary care physicians/NP by making appropriate referrals, follow up calls, and coordination of care. Facilitates patient and family decision making by providing education on palliative care for chronic conditions, end of life care, advance directives, and chronic care management. Provides accurate, bedside, legible documentation in patient’s medical records, using appropriate billing codes. Qualifications: Graduate of an accredited school of nursing Graduate of an accredited Nurse Practitioner Program Current Advanced Practice Registered Nurse (APRN) Licensure Nurse Practitioner certification in Hospice or Palliative Care within 1 year of starting the position Strong computer skills, including Word, Excel, and PowerPoint Minimum of 5 years OR clinical hospice and palliative care experience. 2 years’ experience in the Nurse Practitioner role preferred. Specialized Knowledge and Skills: Strong clinical assessment and critical thinking skills across palliative and hospice populations. In-depth understanding of palliative care principles, hospice philosophy, home health eligibility, and end-of-life care. Working knowledge of LCD criteria, Medicare/Medicaid regulations, coding/billing, and documentation requirements. Excellent interpersonal, written, and verbal communication skills. Knowledgeable of Hospice & Palliative Quality Measures collection, documentation, reporting, and the QAPI process. Ability to work both autonomously and collaboratively in a fast-paced, evolving environment. Compassionate, emotionally intelligent, and sensitive to the needs of patients and families. Proficient in point-of-care technology and electronic medical records. Willingness and ability to maintain productivity and documentation standards in alignment with organizational expectations. Position Summary The Advanced Practice Registered Nurse collaborates with physicians and interdisciplinary teams to provide comprehensive palliative and hospice care in various settings. Essential Functions Provide medical expertise in pain and symptom management, including terminal prognosis evaluation. Act as attending provider in hospice contract beds in collaboration with the hospice physician. Coordinate admissions and discharges for hospice contract beds, including review of terminal diagnoses. Assess patient conditions, develop care plans, implement interventions, and evaluate outcomes. Conduct patient and family assessments, ensuring readiness for transition to hospice or home settings. Perform physical exams, diagnostic testing, treatment, follow-up, and consultations. Prescribe and administer medications and other therapeutic interventions.
