Job Description Join our amazing team of Psychiatric Nurse Practitioners at UCSF Health, where you will serve patients directly within the Emergency Department and the inpatient setting! The psychiatric nurse practitioners will be performing evaluations, placing and releasing legal holds, serving as both consultative and direct patient care role. Evaluating the need for inpatient hospitalization and assisting with dispositioning and after care needs. Responsibilities Work with physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, taking histories, performing physical examinations, peri-operative planning/coordinating with outside providers and primary care providers (PCPs) as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures. Evaluate for and order home health services, durable medical equipment (DME), Department of Motor Vehicles (DMV) certifications, refer for consultations, physical therapy, order radiographic imaging or diagnostic tests, and specialized treatments (e.g. chemotherapy with the approved privilege). Provide patient and family education, arrange follow-up appointments, and engage in any approved privileges after the APP has demonstrated competency through proctoring and FPPE process. OMAG will identify independent practice for approved privileges on the ECHO website and procedural activity will be entered by the provider and monitored via the eLOG system. To evaluate, assess and create a documented plan of treatment per patient’s presenting HPI or established plan of care as defined by the UCSF Medical Staff Bylaws To answer clinically relevant patient medical advice calls or during bedside communications or in coordination with health care professionals, patients, and their families as necessary. To independently, or in a shared visit model, evaluate routine, acute, critical, new patients, or specialty consultations when needed or required and discuss with attending and staff as appropriate and document in the EHR. Any required documentation workflow must be adhered to Be able to interpret and act upon ordered tests, procedures, and radiographic studies To assist in formulating and implementing protocols to improve efficiency in the clinical specialty Effectively communicate with family or patients in person, via telephone and/or via other electronic modalities in a timely manner. Job Description Interested in a dynamic, autonomous practice setting, working with a highly skilled and collaborative APP team? Consider joining our ambulatory care Cancer Center Infusion APP team. The Cancer Center Infusion APP team is responsible for overseeing the care of 50-75 patients per day (depending on size of unit). We cover five Infusion centers providing treatment for Oncology and non-Oncology patients. The Infusion APPs triage multiple issues in real time and ensure continuity of care through collaboration and communication with physicians/surgeons/APP colleagues as well as other clinical and administrative staff. Ultimately, we ensure timely patient care and treatment to meet goals of care, optimize the patient experience and ensure good patient outcomes. In addition to our infusion areas, we cover our three bed Cancer Center Acute Care (CACC) where we manage higher acuity Oncology patients to avoid unnecessary emergency room visits, hospital admissions and facilitate emergency services and admission when necessary. Job Description The summary purpose of the APP Manager is to provide leadership for the health system and advanced health providers in the development and implementation of ongoing quality improvement through an array of regulatory compliance, policies and programs, funding/funds flow/productivity, training and technical assistance, data and information sharing, and partnerships and collaborations. Job Description The University of California, San Francisco (UCSF) Lung Transplant is seeking a full-time Advanced Practice Provider (APP) to join their Advanced Lung Disease Service (ALDS). UCSF is ranked #8 nationally in pulmonology and lung transplant has had significantly higher-than-expected survival rates for post-lung transplant recipients for more than 13 years consecutively! This accomplishment is a result of a great multidisciplinary team that strives to redefine possible at every level of care. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, pharmacists and many others that provide quality care for patients with acute respiratory distress syndrome (ARDS), Alpha-1 antitrypsin deficiency, bronchiectasis, chronic obstructive pulmonary disease (COPD), cystic fibrosis, idiopathic pulmonary fibrosis, interstitial lung disease and pulmonary hypertension. Job Description UCSF Benioff Children's Hospital is recognized throughout the world as a leader in health care, known for innovation, technology and compassionate care. Our 183-bed UCSF Benioff Children's Hospital has more than 150 specialists in more than 40 areas of medicine. The Pediatric Intensive Care Unit (PICU) at UCSF Benioff Children's Hospital provides extraordinary high-level care for children who require continuous monitoring. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. The TCU provides care to patients with a moderate to high complexity of disease such as those recovering from solid organ transplantation, titration of anticoagulant therapy and other infusion, and post-operative specialty surgery care (ENT, neurosurgery). Patients requiring respiratory support including BIPAP, CPAP, high flow nasal cannula and patients requiring mechanical ventilation with tracheostomy in place are also routinely admitted to the TCUP. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents. Primary Care - APP (NP or PA)

Overview ANNUAL SALARY (DEPENDING ON SKILLS/EXPERIENCE): $150,000 - $165,000 Epic Care is a physician-led group and has grown into a dynamic, multi-specialty organization with more than 40 plus physicians located throughout the East Bay Area. Join our team as we innovate both in clinical and practice operations! We are looking for an APP (NP or PA) to work with our Primary Care Physicians. Successful candidate would have a sound clinical base of general medicine knowledge, is energetic, adaptable, collaborative and a team player. Location: San Leandro, Hayward Responsibilities Responsibilities: Assesses, plans, implements, and evaluates care for patients. Initiates and documents appropriate interventions; evaluates and records patient/family response and prescribes therapies as indicated. Performs procedures after appropriate training. Assesses patient/family needs and provides education. Promotes wellness and recommends community resources to meet patient and family needs. Incorporates the developing knowledge of evidence-based research into clinical practice. Developing knowledge of reimbursement procedures and documentation requirements. Completes patient charge capture and medical record documentation in a timely fashion. May provide case management for patients and their families, given limited knowledge of community resources. Other duties as assigned. Qualifications Requirements: Master of Science Degree, Graduate of accredited Nurse Practitioner program or Physician Assistant. Specialty certification required. Baccalaureate in NursingCurrent CA licensure.
