Nurse Practitioner- Palliative/Hospice Care Full Job Description Primary Duties and Responsibilities The Home Health- Nurse Practitioner is responsible for assisting in the expedient multidisciplinary care of patients for Hoag Hospital and its affiliates, coordinating seamless patient care from admission through discharge in the Home Health setting. Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures and facilities the discharge process. Job Description The University of California, San Francisco (UCSF) Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center seeks a full-time Advanced Practice Provider (APP) to treat mechanical circulatory support (MCS), heart transplant, and advanced heart failure patients, among others. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers, caring for patients with advanced heart failure across the continuum of care. Our Advanced Heart Failure & MCS programs are nationally known for cutting edge advanced heart failure care and support in both the outpatient and inpatient setting and offers a variety of support systems (long-term and short-term). The APP serves as primary provider in the hospital setting for these patients and will work closely with the Advanced Heart Failure Cardiologist, Cardiothoracic Surgery and Intensive Care teams along with nursing, case management, social workers and other multidisciplinary teams to evaluate, treat and manage this highly complex patient population in the Intensive Care and Acute Care Units. Responsibilities RESPONSIBILITIES 60% Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 1.1. Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Includes taking histories, performing physical examinations, responding to changes in patient status, performing consults, ordering or furnishing medications/diagnostic tests/treatments/therapies/consultations/referrals/durable medical equipment (DME)/home health services. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. The TCU provides care to patients with a moderate to high complexity of disease such as those recovering from solid organ transplantation, titration of anticoagulant therapy and other infusion, and post-operative specialty surgery care (ENT, neurosurgery). Patients requiring respiratory support including BIPAP, CPAP, high flow nasal cannula and patients requiring mechanical ventilation with tracheostomy in place are also routinely admitted to the TCUP. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents.
