Job Description Summary The successful applicant will be a Nurse Practitioner or Physician Assistant interested in working full -time in the care of patients requiring Palliative Care in our outpatient Adult Palliative Care clinics. The Nurse Practitioner or Physician Assistant will join a busy and thriving Adult Palliative Care team that includes 7 Faculty, 2 Fellows, 4 inpatient APP’s, 3 Palliative Telehealth APP’s, and 2 Adult Palliative outpatient APP in a growing practice. Outpatient Palliative Care services include symptom management, care coordination with the patient’s other medical providers, ongoing discussions regarding treatment choices, and quality of life goals. Our outpatient care team includes doctors, nurse practitioners, social workers, and nurses.
