Wound care certification such as WOCN or equivalent recognized wound care credential preferred Minimum 3-5 years of clinical experience with a strong background in wound care, preferably including in-home or post-acute care settings. Demonstrated leadership experience in clinical settings or wound care programs. BLS certification or higher. Proficiency with electronic health records and digital wound care management tools. Organization: Acute Care Surgery Job Summary: The Advanced Practice Provider (APP) plays a vital role in delivering specialized care and managing the health needs of patients in either inpatient, ambulatory, or a combination of both settings. With advanced training and expertise, the APP focuses on subspecialized care, including chronic illness and disease management, ensuring continuity of care throughout the patient’s healthcare journey, which may span multiple years. KEY RESPONSIBILITIES Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care. In collaboration, provides healthcare services for primary, acute and complex care of patients and manages patients’ overall care, identifying expected outcomes for diagnoses. Orders, performs, interprets and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Hospice Nurse Practitioner

We are seeking a dedicated Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance. This is a less than full-time flex position assisting with Face to Face visits and if available (not required) on the weekends could assist with admissions. Key Responsibilities: Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes. Document clinical findings accurately and attest F2F encounters in patient medical records. Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements. Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians. Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families. Provide clinical recommendations to the IDG based on F2F assessment findings. Conduct assessments in the patient’s place of residence, including home, skilled nursing, or other settings. Notify the hospice agency promptly if unable to complete F2F visits as scheduled. May provide F2F coverage for multiple provider numbers with approval. Organization: PMAC Job Summary: The Pediatric Medicine Acute Care Clinical Staff Leader is a direct supervisory/managerial leadership role and a key member of the Nursing leadership team. The CSL leads a cohort of staff members and actively participates in overall management of the unit, development of clinical practice, and provision of patient care with accountabilities for implementation and outcomes for people leadership, quality, service, compliance, finance, throughput and scheduling. Unit: We care for pediatric patients ranging in age from newborns to 18 years who are being treated for a variety of medical diagnoses. Our patients are unique because we specialize in Epilepsy Monitoring, Neonatal Abstinence Syndrome (NAS), and Peritoneal Dialysis. One of the perks of working on our unit is the wide array of diagnoses and specialized medical needs of our patients. Also, being a 42-bed unit, we are the largest acute care unit at Children’s Hospital. Shift: Night Shift 40 hours a week - Two 12 hour shifts in staffing and two 8 hour administrative shifts . KEY RESPONSIBILITIES Interacts with and contributes to the professional development of their cohort and other staff as needed. Defines standards for quality patient care. Develops, manages and evaluates area based on work team performance.
