In this role, you will use phone and video-based care to provide high-quality, comprehensive follow-up care to patients who are post-hospital discharge. Here's what you'll do: Cover our care needs for our Transitions of Care program. Day to day work includes telephonic outreach (“cold calls”) and scheduled video/phone visits for patients who have recently been hospitalized and discharged. Visits focus on: Medication reconciliation, symptom screening, and ordering of any needed follow-up labs, medications or supplies. You will also navigate conversations with patients to build trust, explain Galileo's care model and connect them to services. Coordinate care across settings — hospital and ED transitions, pharmacies, and community providers — to keep patients connected and moving forward. We work in 4-hour shifts (mornings, afternoons, evenings, overnight on weekdays and weekends). At 24+ hours you will be considered full-time and benefits eligible. Providers are expected to work weekends on a monthly basis, specific schedule will be influenced by total hours committed and patient care demands. We are currently looking for a Nurse Practitioner with an active license in NJ who is available to work 24-36 hours/week. Virtual, multi-state palliative care is a novel concept and we are looking for a highly motivated, innovative, experienced palliative care nurse practitioner to help build the foundation of our clinical team. Our ideal NP is an expert palliative clinician with excellent communication and team-building skills who is adaptable to a continuously evolving startup environment. Skills and experience: Required: Must possess a current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state. Must be willing to obtain additional state licenses as required by the business. Provide consultative visits to patients empaneled to the RNs, including but not limited to: advanced symptom management; complex goals of care; advanced disease education; prognostication; medical orders for home health, DME, medications, and hospice transitions. CCM/RPM The medical assistant works in collaboration and continuous partnership with chronically ill patients and their family/caregiver(s), clinic/hospital/specialty provider and staff, and community resources in a team We are seeking a full-time, bilingual, and remote Medical Assistant (MA) to join our rapidly growing team at Preventel Health performing Chronic Care Management and Remote Patient Monitoring (RPM) by telephonically delivering health and wellness calls to assigned patients. Responsibilities: Remotely providing basic patient coaching and care to improve patient outcomes. Coaching and educating patients on improving their Chronic Conditions, preventive care and physician directives Updating and managing patient care plans based on assessment of patients’ needs and physician directives Schedule appointment scheduling on behalf of physicians Telephone outreach to English and Spanish-speaking patients Facilitate CCM program enrollment Facilitate RPM- remote patient monitoring enrollment Monitor patients outcomes Clear explanation of the benefits of chronic care management Patient eligibility verification Concise and accurate documentation Cultivate and support the primary care providers with timely communication, inquiry follow-up, and integration of information into the care plan regarding transitions-in-care and referral · Serve as the contact point, advocate, and information resource for patients and their care team. · Work with patients to plan and monitor health and social needs · Develop a care plan with the patient, family/caregiver(s) and provider · Monitor adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner and facilitate changes as needed · Create ongoing process for patients and family/caregiver(s) to determine and request the level of care coordinates support they desire at any given point in time Requirements: Medical Assistant certification Fluent in Spanish Knowledgeable using Excel spreadsheets Computer with internet COMPENSATION: Based on a base plus incentive model.

This role is for a per diem Nurse Practitioner who will be able to work evening and weekend shifts, up to 8 hours per month. We are currently looking for a Nurse Practitioner with an active Texas state license. Please note, during the initial month of onboarding, we will require the ability to work up to 10 hours/week.
