JOB SUMMARY: Assists provider in patient care activities conducted in the clinic and performs selected procedures under the direct supervision and request of the provider. Reporting to the Clinical Operations Supervisor, the Medical Assistant performs functions to assist in providing medical care and treatment to patients in the clinic setting. PRINCIPAL RESPONSIBILITIES AND DUTIES: Welcomes patients by greeting them, in person or on the telephone; answering or referring inquires. Perform some front desk duties (i.e. checking in patients and scheduling patient appointments. Prepares referrals orders in EMR, IRIS and County Care Portal. Obtain immunization records from I-Care. Give instructions to patients upon provider’s directive. Ensure all medical reports, labs and information is filed and available in the patient’s medical records prior to their appointment. Keep exam rooms clean and stocked with adequate medical supplies; maintain instruments and diagnostic equipment, and sterilizing medical instruments. Take telephone messages and provide feedback and answer to patients/providers/pharmacy calls. Triage and process messages from patients and front office staff to provider’s. E-scribing medication to the pharmacy. Prepares schedule for daily huddle with Care Team. Signs off on huddle completion in EMR. Maintain all logs and required checks (i.e. STI log, refrigerator temperatures, expired medication, oxygen etc.). Complete and fax STI report to the Board of Health. Generate sample medication label for the patients when samples are given out from the clinic. Faxing documents, scanning and verifying patient documents in EMR. Keeps supplies ready by inventorying stock, placing orders and verifying receipt of orders. Schedules and documents relevant information in the patient’s medical record, point of care test results and procedures performed. Perform CLIA Waived testing (i.e. pregnancy test, urine dips, Strep Screening, Influenza, PT/INR’s, glucose testing, and Hemoglobin, and Hemoglobin A1C, etc.) Document and maintain immunizations records in EMR. Accountable for panel metric performance in admission prevention, readmission prevention, and transitions of care metrics. Owns overall care coordination for assigned patients, functioning as the primary point of contact and ensuring alignment, accountability, and follow-through across the care team. Manage transitions of care episodes for patients on their panel, including timely completion of structured clinical assessments to identify post-discharge needs, medication reconciliation on behalf of the PCP, and addressal of identified needs directly or via collaboration with other team members. Collaborates patient’s PCP, family/caregiver, Social Worker, Behavioral Health Specialists, and other care team members, as needed to evaluate the individual's needs, goals, and plan of action and ensure care plan progression. Ensure timely documentation of key clinical assessments after admissions, while balancing in-center care team planning meetings. Lead in-person interdisciplinary care planning meetings to ensure effective care coordination and management between providers visits. Perform timely nursing assessments and provide patient education for chronic condition management and transitions of care. Educate patients and families, empowering them in their care, and advocating for their needs. Document visits in electronic health record according to internal standards Other duties as assigned. What are we looking for? Current RN license in assigned state is required; Bachelor degree in nursing preferred. Minimum of 6-8 years nursing experience. Certified Case Manager (CCM) required, or willingness to obtain within 12 months of hire, unless candidate has 2-3 years of relevant care/case management experience 2+ years experience in transitional nursing, emergency room nursing, care coordination, discharge planning, or home health is strongly preferred. Demonstrated skill in motivational interviewing, patient activation, time management, and navigating community and social resources. The Family Medicine team is uniquely trained to care for patients as a whole person, regardless of age or sex. The cornerstone of Family Medicine is an ongoing, personal relationship between caregivers and patients to achieve the best possible health outcomes. Caregivers and patients work together to achieve the best possible health outcomes. A caregiver in this position works Monday through Friday from 8:00am to 5:30pm. Once a week there will be coverage needed until 7:30pm. Rotations include 1 weekend a month from 8:00am to 12:00pm. A caregiver who excels in this role will: Assist with performing screenings and treatments by gathering and updating information related to health maintenance, identifying medication changes, updating allergy information, collecting specimens, performing tests and documenting patient care. Monitor and communicate changes in patient condition. Maintain equipment, medical supplies and examination areas. Perform administrative/clerical duties, such as filing, reception, scheduling, data entry and patient registration. Minimum qualifications for the ideal future caregiver include: Graduate from an approved medical assisting program OR military training as a hospital Corpsman (HM), Combat Medic (68W) or Medical Service Technician (4N0X1) Completion of a clinical externship OR extensive clinical experience Basic Life Support (BLS) certification through the American Heart Association (AHA). May require extended periods of standing, walking or sitting. Good visual acuity through normal or corrected vision.
