PRIMARY TASKS AND RESPONSIBILITIES: o Assess the physical and psychosocial status of patients by means of interview, health history, physical examination, and diagnostic studies. o Write prescriptions for: medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and care post-discharge in accordance with written practice protocols. o Based on findings, determine eligibility of patients for entry into clinical trials. o Interpret and evaluates findings of studies / tests. o Relay appropriate information regarding patient care to the collaborating physician. o Administer therapeutic measures / obtains specimens as prescribed. o Dictate follow-up letters to referring physicians with summaries of treatment, response and plan. o Dictate or writes patient history, admissions, progress notes, and discharge notes in a timely manner. o Assist in the management of family dynamics and coping mechanisms during acute and chronic phases of patients’ care. o Conduct or participate in daily multi-disciplinary rounds for assigned patients to ensure patients are receiving appropriate and quality care. o Provide on-call coverage and performs hospital rounds as needed. o Triage patients to determine urgency of physician evaluation. o Document and reports care in an accurate, timely manner using appropriate forms and records. o Demonstrate a willingness to learn and to remain flexible in the changing health care environment. o Assist staff in times of crisis or emergency. o Meet with the public outside the institution and efficiently market and promote cancer treatment. o Determine work priorities, assign work and insure proper completion of work assignments. o Comply with all federal and state laws and regulations pertaining to patient privacy, patient’s rights, personnel law, safety, labor, and employment law. Adheres to FCS and departmental policies and procedures, including IT policies and procedures and Disaster Recovery Plan. o Other duties as requested. o Keep work area and records in a neat and orderly manner. o Maintain all company equipment in a safe and working manner. o Maintain and ensure the confidentiality of all patient and employee information at all times as required by the HIPAA regulations. o Assist in training new employees to related job duties. o Will be expected to work at any FCS location to help meet FCS business needs as directed by collaborating physician. o Will be expected to work overtime when given sufficient notice of required overtime. EDUCATION/CERTIFICATIONS & LICENSES: o Graduate from an accredited Nurse Practitioner program, master’s degree in nursing or post-masters nurse practitioner certificate program o 0 - 3 Years Licensed as Nurse Practitioner or Physician Assistant o Current license/registration to practice in the state of Florida, prescriptive privileges or eligible to apply o Certification from American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the Oncology Nurses Certification Corporation (ONCC). o BLS certified EXPERIENCE: o Leadership skills necessary to direct others toward objectives that contribute to the successful treatment of patients. The establishment of Collaborative Practice Agreement Chapter 257 of the Laws of 1988 of New York State which took effect April 1, 1989, authorizes Nurse Practitioners certified by the State Education Department to use the title "Nurse Practitioner" [for Nurse Practitioner job description, see appendix] and engage in the expanded practice of nursing, which may include the performance of primary health care services and the issuance of prescriptions. Practice agreements and practice protocols must be established and maintained in the practice settings of the Nurse Practitioner and collaborating physician where they shall be available to the Department for inspection. 2. Practice Protocols Practice protocols shall identify the area of practice to be performed by the Nurse Practitioner in collaboration with the physician and shall reflect accepted standards of nursing and medical practice. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. . The Surgery Specialty Clinic care team includes doctors, nurse practitioners, physician assistant, registered nurses, licensed practical nurse, medical assistants, and physicians in training. The team works together to provide a multidisciplinary and holistic approach to best serve the patient and the family. The Surgery Specialty Clinic Team provides an enjoyable atmosphere for staff that includes ongoing professional growth and development. Duties To Include Applies nursing theory to the assessment, diagnosis, treatment, and evaluation of patient responses to health and illness. Telephone triage- asking questions to gauge the patient's condition and recommend the best course of action. This could mean advising the caller to seek emergency care, treat their condition at home, or make an appointment to see a provider. Assesses patient health care preferences and expectations- Provide education to patients preparing for tests, pre-operative information, and reviewing medications Collaboratively coordinates the continuum of care across settings. Assist with coordinating clinic and radiology appointments at UI Health Care and locally as appropriate Develops patient care plan based on patient condition, age, and psychological, educational, and socioeconomic conditions that prescribes interventions to attain expected outcomes - Implements patient care plan according to patient priorities, which includes interventions identified through nursing diagnoses. Documents/reports interventions and patient/family response. Collaborates and integrates contributions of patients and families into multidisciplinary patient care plan. Work with the patient and family to ensure patient needs are met following procedures/surgeries Implements nursing interventions according to department, division, and/or unit policies, procedures, and protocols to maintain safe environment for patients, visitors, and staff. Follows universal precautions. Review the patient’s medical record(s) to provide accurate information related to their concern and reason for seeing the physician. Review previous testing done and obtain records and images related to clinic visit. Percent of Time: 100% Salary: $61,183 minimum at 100% effort to commensurate. Salary will be based on years of experience and percent effort Schedule: 100% effort : Monday through Friday, 0730-1600, 0800-1630, and 0830-1700 or until clinic close. No weekends, no holiday, no call. Education An Associate’s Degree in Nursing, a Diploma in Nursing, a Baccalaureate Degree in Nursing, Professional Masters of Nursing and Healthcare Practice (MNHP), MSN/Clinical Nurse Leader or a Master’s Degree in Nursing (MSN, MA) is required Required Qualifications Excellent written and verbal communication skills are required.
