Overview An Advanced Practice Provider is a direct patient care provider licensed in the State of Florida and/or Georgia and provides medical services to patients as defined by their scope of practice and under the supervision of a physician licensed to practice medicine in the State of Florida/Georgia. Provides patient care with advanced knowledge of subspecialty service. Clinical Area: Subspecialty Outpatient and/or Inpatient Setting, may perform procedures/consults, less than 50% time spent in OR, and/or works in the NICU. Responsibilities • Provides timely and appropriate referrals to patients in need of further medical care and services. Responds to consults, schedules procedures and handles discharge planning, as appropriate. Educates and counsels patients/families regarding their health care, including routine health maintenance, as applicable. May provide education and instruction to residents and students. • Performs or assists with procedures of higher complexity in clinic or inpatient unit. Monitors and manages invasive and/or medical devices and placement of devices within scope of practice. May manage care of pre/post-operative and pre/post-procedural patients. • Formulates comprehensive health management plans for high acuity, more complex patients in collaboration with physicians, pharmacists, and other medical professionals to include progress notes, narrative discharge summaries, consults, etc. Facilitates patient participation in making health care decisions. • Enters pertinent clinical information in the patient’s medical record in a timely and accurate manner and in accordance with established compliance billing guidelines. Documents and closes open encounters, MyChart messages, and other EMR communications in accordance with institutional requirements. • Call coverage may be required specific to departmental needs. • Obtains patient histories, performs physical examinations and/or diagnostic and therapeutic procedures, as well as orders, interprets, and evaluates diagnostic tests for patients with high level of complexity. Description A Nurse Practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner in a specialty provides acute/primary health care in accordance with written policies approved by nursing and medical administration and manage a clinical program throughout the hospital continuum. Responsibilities Responsible For Patient care as outlined within collaborative practice agreement. Principal Duties and Responsibilities 1. Clinical Practice a. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. b. The establishment of Collaborative Practice Agreement Chapter 257 of the Laws of 1988 of New York State which took effect April 1, 1989, authorizes Nurse Practitioners certified by the State Education Department to use the title "Nurse Practitioner" [for Nurse Practitioner job description, see appendix] and engage in the expanded practice of nursing, which may include the performance of primary health care services and the issuance of prescriptions. Practice agreements and practice protocols must be established and maintained in the practice settings of the Nurse Practitioner and collaborating physician where they shall be available to the Department for inspection. 2. Practice Protocols Practice protocols shall identify the area of practice to be performed by the Nurse Practitioner in collaboration with the physician and shall reflect accepted standards of nursing and medical practice. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. . Staff must demonstrate flexibility for reassignment to units providing care to varied populations including male and female children, adolescents and adults as well as dually diagnosed adults.
