Job Title: Licensed Practical Nurse (LPN) - ElderONE Part Time - Varied) Department: Homecare Location: ElderONE - North Park Pace Center 355 N Park Dr, Rochester, NY 14609 Hours Per Week: 20 Schedule: Varies SUMMARY: The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans. This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided. RESPONSIBILITIES: Conducts and documents a thorough observation of each patient’s medical status upon admission and throughout the patient’s course of treatment. Assists in the implementation of the patient’s treatment plan under the direction of the RN. Provides routine nursing services for patients as directed by the RN. Complies with the evaluation, treatment, and documentation guidelines of the department. Completes required documentation in an accurate and timely manner. Provides regular patient status updates to the appropriate personnel. Attends and participates in department/facility meetings as required. Acts in compliance with hospital, regulatory, and professional standards and policies. Maintains clinical competency by pursuing continuing education opportunities. Performs other duties as assigned. REQUIRED LICENSURE / CERTIFICATION SKILLS Graduate from an accredited School of Practical Nursing. Current LPN licensure in State of New York. Department/Unit: Ambulatory Surgery Work Shift: Weekend_Day (United States of America) Salary Range: $43,599.00 - $56,679.00 PreOp unit is seeking to fill Patient Care Associate/PCA. This position is for every weekend (Saturday & Sunday) from 6am - 6:30pm. The weekend track PCA will also carry out clerical duties such as answering the phone, putting together patient charts, and scheduling transportation for patients. The Patient Care Assistant (PCA) will work under the direction of the Registered Nurse. During the assigned work period, PCAs will collect patient data, complete personal care of the patient, perform point of care testing such as whole blood glucose, turn & position and/or mobilize patients, and assist with caring for the needs of assigned patients with a focus on progress toward discharge, including during transitions within the acute care stay.
