Nurse Practitioner - Pulmonology

This position works in collaboration with physicians to provide complete physical examinations, diagnosis, and treatment of both chronic and episodic disorders. Assigned principal responsibility for primary health care of patients. Seeks the expert opinion or advice of a physician whenever the incumbent decides a case falls outside the scope authorized by policies and protocol. The Nurse Practitioner typically remains responsible for the patient throughout the course of treatment, provides the physical examination, the diagnosis, and the health care plan, determines when the patient has recovered, and releases the patient. The Nurse Practitioner may refer the patient to a physician for consultation or to specialized health resources for treatment, but retains responsibility for clinical management as well as any subsequent modification. Responsibilities Assesses the physical condition of a group of patients by performing physical examinations including ordering, interpreting and evaluating diagnostic tests and obtaining medical histories. Assumes ongoing responsibility for each patient from discovery of the disorder to recovery. Formulates the medical plan and prognosis based on patient's condition. Consults with physician(s) regarding patient health care plans and treatment. Orders, interprets, and evaluates diagnostic tests to identify and assess the patient's clinical problems and health care needs. Performs selected invasive procedures for therapeutic and/or diagnostic reasons within scope of validated competence. Initiates and/or recommends medical drug therapies or other forms of related therapeutic treatment and procedures in accordance with approved protocols/policy. Complies with medical records policies regarding the complete and accurate documentation of patient care. Initiates and maintains required records and legal documents. Participates in quality improvement activities. Counsels patients, families and significant others about health and illness, and promotes health maintenance. Establishes and documents the health care plan and prognosis, maintaining records of each case and evaluates the effectiveness of the plan. Initiates drug therapy, taking full responsibility for such therapy. Consults with a physician prior to implementing the therapy, as necessary. Determines when the patient has recovered from the disorder and releases the patient. Qualifications Education: As required by licensure. Experience: Clinical experience requirements are dependent upon the specific position, and include a minimum of 2 years of experience in a related field. Skills/Knowledge: Basic familiarity with computers preferred. Licensure Current applicable state RN license required. National Certification as a Nurse Practitioner (NP) required. Current applicable state license as a Nurse Practitioner with Prescriptive Privileges (NP-PP) required. Current certification dependent upon the specific position to include Basic Life Support (BLS) from the American Heart Association. Pay Range USD $74.84 - USD $97.56 /Hr. How you make a difference The Certified Medication Technician is responsible for administering prescribed medications under the supervision of a registered nurse. This role includes documenting medication administration, adhering to infection control protocols, and ensuring compliance with safety and regulatory standards. The technician participates in audits and data collection to support performance improvement efforts. They monitor for adverse reactions and report them promptly while maintaining medication supply levels. The technician also ensures patient confidentiality and compliance with HIPAA regulations. Key Responsibilities Administer medications under RN supervision in compliance with policy and regulations Document medication administration and reasons for non-administration in the MAR Follow infection control procedures and safety protocols during medication delivery Participate in audits and report adverse reactions to supervisors Maintain medication supplies and uphold HIPAA confidentiality standards Qualifications & Requirements EDUCATION High School graduate or equivalent. EXPERIENCE 1 year related experience in a medical environment preferred. LICENSES/CERTIFICATIONS Must be certified as a Medication Technician in the state within which services are provided and have current CPR Certification. RN Staff Nurse - Vascular Access

Responsibilities Responsible for patient care within an assigned department/unit and participates as a member of the health care team in cooperation with and under the direction of the clinical manager or designee as defined by the relevant Standards and Scope of Practice for the Registered Nurse. The RN Staff Nurse assesses, analyzes, plans, implements and evaluates patient care following established patient care standards for Legacy Health System including those for patient assessment and transfer. The Vascular Access Nurse provides consultation for providers and nurses for the most appropriate vascular access device based on patient assessment and the treatment plan. Places vascular access devices such as: peripheral intravenous catheters (PIV), Peripheral Inserted Central Catheters (PICC), Midline catheters and Intra-osseous catheters. Accesses and de-accesses implanted ports. Assesses and manages central venous access devices including routine dressing changes, cap changes and intervenes to maintain patency of central venous access devices. Serves as a resource for the health care team in the intravenous access, placement, management, and patient/family education for care and maintenance. Service area is both inpatient and outpatient. Qualifications Education: As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration. Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized. Thorough knowledge of care and skill in placement of peripheral IV catheters; thorough knowledge of care of central IV access devices and PICC competency as determined by Nurse Manager or designee. How you make a difference As a Nurse Practitioner MAT, you will deliver comprehensive medical care to patients, operating within established policies and procedures and in alignment with state regulations, under the guidance of the Site Medical Director and fellow site Physicians. Your role involves meticulous documentation of provided services for monitoring purposes and promptly communicating any schedule changes or coverage needs to the Medical Director and site director to ensure uninterrupted medical services for patients even during your absence. Key Responsibilities Conducts thorough medical examinations of referred patients upon reporting to the assigned facility, adhering to clinic treatment policies and all relevant state and federal regulations. Provides guidance and training to medical staff, participates in case conferences, and collaborates with clinic personnel on treatment plans while ensuring accurate and timely documentation of patient progress in SOAP format. Responds promptly to health emergencies following standard protocols, reviews records as necessary, and utilizes available resources for problem resolution before considering off-site referrals. Performs comprehensive medical assessments, including history-taking and physical examinations, to diagnose opioid dependence and coexisting medical or psychiatric disorders, making appropriate referrals for further treatment. How you make a difference The Nurse Practitioner provides on-site medical services by examining referred patients, conducting clinics, rounds, and responding to urgent and emergency situations. The role delivers ongoing medical care by diagnosing conditions, prescribing treatment plans, managing chronic care, and providing Medication Assisted Treatment services, including induction, follow-up, and medication management. The position documents care, orders and interprets labs, adheres to established policies and formularies, and supports continuity and standards of care within the facility. Key Responsibilities Examine patients, conduct sick call, chronic care clinics, and infirmary rounds, and respond to health emergencies. Diagnose conditions, prescribe and manage treatment plans, and provide ongoing medical and MAT-related care. Conduct MAT assessments, inductions, follow-up appointments, and manage medication continuation and titration. Order and interpret laboratory tests and drug screens and utilize appropriate in-house and referral resources. Document patient encounters, maintain confidentiality, and comply with established policies, procedures, and standards of care. Additional Details Candidates should have experience providing independent primary and urgent care, including patient assessments, diagnosis, prescribing, chronic disease management, sick call, and emergency response. They should also be experienced in substance use disorder treatment and Medication-Assisted Treatment, including MAT evaluations, medication induction, continuation, dosage titration, follow-up care, withdrawal management, and relapse-prevention support. Experience ordering and interpreting labs and drug screens, coordinating referrals, maintaining accurate clinical documentation, and working within established formularies and treatment protocols is important. Correctional healthcare, reentry-focused care, Medi-Cal/CalAIM programs, and collaboration with multidisciplinary discharge-planning teams are strongly preferred. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage.
