Compensation Pay Range: $38.20 - $77.39 The role requires IV clinical expertise, ability to assess veins and make recommendations to providers for other access devices based on the vein assessment, critical thinking, collaborative practice as a venous access nurse within a cancer center, line care maintenance and troubleshooting, knowledge of vein preservation principles and techniques, excellent communication skills and an understanding of the principles and practices of clinical research. PRIMARY DUTIES AND RESPONSIBILITIES: A. Delivers expert, evidence-based nursing care to patients and families. 1. Implements nursing process:  Assess veins and access needs and makes recommendations to providers accordingly.  Assesses patient and family needs.  Evaluates home line care routine and supports patient to achieve optimal outcomes.  Analyzes assessment data to develop and support clinically accurate diagnoses.  Plans care by establishing goals with measurable outcomes.  Implements the plan of care.  Evaluates the patient's response to interventions. 2. Documents care according to Institute and regulatory agencies' requirements. 3. Administers medications relevant for care of the venous access patient and/or specific to Imaging protocols and performs procedures safely and efficiently in accordance with Institute policy. May provide specialized care of protocol drug patients in various settings. 4. Evaluates educational and psychological needs of patient and family and incorporates identified needs into the plan of care. 5. Educates patient and family regarding various aspects and treatment modalities of disease, including vein choice, vein preservation techniques, and line care if applicable. 6. Recognizes and responds to emergency situations appropriately. Maintains BLS certification and or ACLS certification. 7. Identifies appropriate community resources to promote continuity of care and makes appropriate referrals. 8. Complies with accepted standards of professional nursing practice, as well as with all licensure/registration requirements applicable to the nursing role. B. Establishes rapport with patients and families from a variety of cultural, ethnic, and religious backgrounds. 1. Interacts with patients and families in a caring and compassionate manner respecting the need for privacy and confidentiality. 2. Develops approaches to problem solving which meet the individual needs of the patients and families. 3. Demonstrates the knowledge and skills necessary to provide age-specific care and support to the populations served. 4. Outpatient care is conducted at the Stanford Children’s Health Clinic in Walnut Creek and inpatient care is conducted at John Muir Hospital. This satellite practice provides an excellent opportunity to practice with a close knit group of community physicians as well as within the larger community of Stanford Children’s Health in a collaborative and supportive team practice. This is a fixed term position for 6 months. General surgery patients range from premature infants to young adults. Common conditions seen include emergency, acute, and chronic surgical conditions, infant and congenital surgical conditions, colorectal and intestinal conditions, tumors, and vascular lesions and vascular associated syndromes. The general surgery department also has numerous specialty outpatient programs including aerodigestive and airway reconstruction center, adolescent bariatric surgery, chest wall program, colorectal and bowel management program, intestinal failure and short gut program, and pilonidal disease program. This position offers the opportunity to be involved in the whole spectrum of surgical patient care from outpatient clinical evaluation, specialty clinic evaluation, and inpatient rounding and management of post/preoperative patients. Outpatient responsibilities include staffing independent NP/PA clinics, assisting with surgeon clinics, assisting with patient advice and questions, family and device teaching, and case management. Inpatient responsibilities include collaborating with the physician team to round on general surgery inpatients, new patients consults, address calls from inpatient team members, and coordinating discharge needs. An NP/PA shall consult with a physician regarding any task, procedure or diagnostic problem which the NP/PA determines exceeds his/her level of competence or shall refer such cases to a collaborating physician. Scope of Practice of the NP/PA: Evaluates and treats patients with acute, chronic complaints and health maintenance concerns related to specialty, according to written standardized procedures. Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching and supervision of the delivery of care to meet the needs of the patient. • Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals through regular home visits in compliance with regulatory and VNS Health requirements. Assesses the patient’s condition, monitors the effect of the care plans on the patient/family health status, informs physician and other team members of changes in the client's needs and updates the plan of treatment, as needed. • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • For NFP Only • Establishes, implements, and revises a plan of care with input from the client and from appropriate multidisciplinary health team members (i.e., social worker, physician) based on goals mutually acceptable to the client.
