This position is eligible for $5,000 retention bonus. Job Summary The Providence Center ACT Engagement and Diversion Team is looking for a motivated, team-oriented Registered Nurse, to help assess and facilitate the tasks and duties listed below for adults with co-occurring conditions, medical complexities, and who are also dealing with unstable living arrangements. Experience with the chronically homeless and addictions populations a plus, and within our new "CCBHC" (Certified Community Behavioral Health Clinic") capacity. Duties & Responsibilities Triage and assess assigned consumers on the team. Assess and evaluate client health care data collected through interviewing, observation, record review and team input. Develop an individualized consumer plan of care. Perform integrated care in accordance with appropriate medical protocol. Identify expected outcomes of care, individualized to the client, with the ultimate goal of improving client’s overall health. Implement the consumer plan of care. Evaluate the consumer's response to the plan of care and update the plan based on consumer's response. Provide support to consumers regarding daily living issues, conducting community and home visits as necessary. Instruct consumers and families concerning the disease process, plan of treatment and rehabilitation course, using available teaching materials when appropriate. Educate consumers on the various medications on the expected effects and side effects. Question any aspect of the consumer's treatment regimen which endangers the level of wellness. Administer medications as per prescriber orders (Injections and PO). Implement appropriately prescribed medical regimen while following the 6 rights of medication administration. Monitor lab results and complete related documentation Ensure that externally prescribed medications (from outside providers i.e. PCP) are accurately documented in EMR. Liaise with treatment team and pharmacy regarding medication changes to ensure clients receive accurate and most current prescribed medications. Coordinate activities regarding consumer hospitalization. Follow up with clients post-hospital discharge to provide support and education regarding recommendations resulting from inpatient stay. Complete medication reconciliations, review hospital discharge paperwork, and communicate any changes or significant findings to treatment team, including MD. Collaborate with the staff members in providing care to consumers. Perform recordkeeping in accordance with Agency requirements. Complete initial and annual assessment forms. Maintain cooperative relationships with consumers, staff and the public. Communicate with various local agencies, medical facilities, and other departments within TPC, including pharmacy. Attend trainings, case presentations and conferences as needed. Perform duties in accordance with departmental goals, objectives and standards. Serve on appropriate committees. Partake in training new hires and student Nurses. May be asked to share on-call with other nursing and program staff. Requirements Associate’s degree in Nursing required; Bachelor's degree from accredited four-year College or University preferred. Bilingual preferred. Rhode Island Registered Nurse (RN) license or Compact License required. Experience in Human Services or Outpatient setting preferred. Experience working with psychiatric and substance use disorder patients also a plus. Position may require driving. Must have valid driver's license, registration, current inspection and insured automobile. Responsibilities What You’ll Do Provide community-based nursing care coordination across CFCM, Shared Living (SLA), and BHDDH programs Conduct comprehensive health assessments and develop individualized nursing care plans Coordinate care across medical, behavioral health, specialty providers, hospitals, and community services Serve as a clinical resource to Shared Living Providers, offering guidance, training, and ongoing support Monitor chronic conditions, medications, treatment plans, and health outcomes Review hospital discharges, ER visits, and clinical recommendations to ensure timely follow-up Deliver education on health promotion, medication safety, infection control, and chronic disease management Ensure compliance with BHDDH, EOHHS, Medicaid, HIPAA, and agency standards Participate in interdisciplinary team meetings, quality reviews, and incident management processes Advocate for individuals served and promote person-centered, community-based care Qualifications What We’re Looking For Active Registered Nurse (RN) license (Rhode Island required) Bachelor’s degree in Nursing required 3–5 years of nursing experience , preferably in community health, IDD, behavioral health, or care coordination CPR/First Aid certification (or ability to obtain) Valid driver’s license (Class C or D) Key Skills for Success Strong clinical assessment and critical thinking skills Ability to manage complex care coordination across multiple providers Excellent communication, documentation, and interpersonal skills Comfort working independently in community-based settings Experience supporting individuals with IDD, autism, behavioral health, or chronic medical conditions Strong commitment to advocacy, person-centered planning, and regulatory compliance Ability to respond to clinical concerns, crises, and changing health needs Why Join Us This is a highly rewarding nursing role for someone who wants to move beyond the traditional clinical setting.
