Performs the comprehensive assessment and develops/implements and evaluates the plan of care as identified on admission and subsequent home visits. Provides appropriate and through education to patients and their caregivers as it relates to the plan of care. When assigned, serves as case manager to coordinate multidisciplinary services per the plan of care. Maintains acceptable productivity standard as determined by agency leadership. Participates in multi-disciplinary team meetings in order to share information and assist with patient management. Participates in staff and administrative meetings. Participates in quality and performance improvement activities. Perform other duties as assigned. Qualifications: Graduate of an accredited nursing program and has experience as a Registered Nurse Valid RN License from Virginia or reciprocal compact state required. BSN is preferred; all Registered Nurses with an Associate's Degree in Nursing will be required to obtain their BSN within five years of hire.
