The R.N./L.P.N. provides supervision of staff and will safeguard the health, safety, and welfare of all Residents/residents under their care by following applicable laws, regulations, and established nursing policies and procedures. Provides quality nursing care to Residents/residents in an environment that promotes their rights, dignity, and freedom of choice. 2. Provides supervision to G.N.A’s and all subordinate staff which includes checking their work to ascertain that assignments have been completed. 3. Assign all G.N.A’s and other subordinate staff to Residents. Assign break times and all regular and special assignments. 4. Administer all parenteral, intramuscular, and sub-cutaneous injections. 5. Administer medications within the scope of practice of the R.N./L.P.N. Licensure. Maintain a current report of narcotics received and used. 6. Review daily the documentation of the dispensing of the controlled substances and narcotics. 7. Place pharmacy orders, for and administer all newly prescribed medications and document. 8. Carry out direct contemporaneous charting in your shift. Chart detailed monthly evaluations of each Resident that reflect his/her condition and progress. 9. Responsible for all nursing care of assigned Residents while on duty. Must notify appropriate persons if there is any significant change in a Resident’s condition or any transfer to a hospital. 10. Ensure that Resident care plans are being followed and asses each Resident’s status in accord with their care plan. 11. Round with physicians when needed. 12. RN’s to assess residents for change in clinical condition and prn. 13. LPN’s to monitor residents for change in clinical condition and prn. 14. Administer or supervise all treatments prescribed by physicians including but not limited to pressure ulcer care, Foley catheter care. 15. Supervise serving of prescribed diets and fluids intake and immediately report any issues to the physician and DON/ADON. 16. Must be knowledgeable of individual resident focused care plans and support the care planning process by reporting specific information and observations of the Resident’s needs, preferences and report any behavioral changes. 17. Provide wound care when needed. 18. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality. 19. Assure that established infection control and standard precaution practices are maintained when providing care. Follow established safety precautions when preforming tasks and using equipment and supplies. 20. Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons. 21. Maintains the comfort, privacy and dignity of Residents/residents and interacts with them in a manner that displays warmth, respect and promotes a caring environment. 22. Answer and respond to call lights promptly and courteously. 23. Assist with new admissions, re-admissions and assist with the transfer of Residents to different rooms within the facility. 24. Communicates and interacts effectively and tactfully with Residents/residents, visitors, families, peers and supervisors. 25. Attend and participate in departmental meetings and in-services as directed. 26. Communicate with other employees in a private and respectful manner. 27. Work and be proficient with certain computer systems designated by the company. Supervisory Responsibility This position has direct supervisor responsibilities for Geriatric Nurse Aides and other personnel assigned. Work Environment This job operates in a health care setting. This role requires regular walking to various locations around the facility while pulling or pushing carts. This position works mostly in the Resident care areas as well as in occasional outside weather conditions. This role may also come into contact with patients who may have contagious illnesses. Physical Demands while performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping on a regular basis. The employee must frequently lift and/or move objects and patients weighing over 75 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Position Type and Expected Hours of Work This position is part of a health care facility that is open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather will be required. Travel No travel is expected for this position. Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. Ensures patient awareness related to transplant and treatment modality options. Assists in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners. Performs assessment and identifies barriers of the Home Dialysis candidate’s home environment and partner/ family readiness and ability to perform dialysis treatments in the home. Trains home dialysis patients and / or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies through a formal standardized Home Dialysis Training Program. Participates in education and quality improvement projects at the facility and area level as directed by Supervisor. May serve as a Preceptor to new employees. Required to complete CAP requirements to either maintain or advance. Performs all other duties as assigned by Supervisor. The responsibilities of this job include, but are not limited to the following: Locating patients, obtaining accurate vital signs by visual and auditory measures, triaging patients, assessing patients’ medical condition, and preparing patients for physicals and medical treatment by Physicians and Nurses; Understanding and using the electronic medical record system; Ordering lab, x-ray, and medical supplies as directed; Notifying the Nurse and/or Physician when the patient is ready for exam while communicating symptoms and relaying questions about lab or x-ray needs; Assisting Nurses and Physicians with various forms of patient treatment and care; Handling Physician and Nurse requests with respect and in a timely manner; Performing lab duties as assigned; Performing phlebotomy procedures; Sterilizing and setting up instruments; Receiving and storing supplies; restocking treatment rooms as needed; Reporting to the Charge Nurse before leaving at the end of the shift; Following ancillary guidelines as deemed necessary by the Charge Nurse, Nurse Supervisor, DMS, and Physician; Adhering to established policies and procedures; Maintaining cleanliness of the treatment area and completing biological cleanup in accordance with OSHA guidelines and other maintenance assignments as directed; Washing, drying, folding, and restocking laundry; maintaining the cleanliness of the washing machine and dryer lint trap; Attending staff meetings as scheduled by the Nurse Supervisor or DMS; Operating, using, and maintaining medical and office equipment as trained; Completing assigned checklists and Policy Manager tasks within the assigned shift; Being available to assist as needed, as breaks and mealtimes may be interrupted at any time to provide necessary patient care or to maintain center operations; Fostering teamwork and ensuring a positive and professional atmosphere; Providing positive, warm, and friendly service in all interactions; Understanding and performing all duties as assigned.
