As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Familiar with the concepts and needs of patients/families who are facing death and dying. This is a Full Time position doing Home Visits for Hospice Care. RN certification in OH required. Our Business Hours are Mon-Fri, 8AM-4:30PM. Oncall rotation for evening/weekends as needed. Our agency covers a 50 mile radius. Candidates must be able to see patients within this area.
