Responsibilities The Patient Throughput Coordinator will work with multiple entities to find and surmount the barriers to discharge of patient. This position acts as the designated liaison who facilitates and coordinates the transfer of patients into and within University Hospital. Works with referring agencies, hospitals, clinics, and the emergency department to ensure safe, timely transition of patients and clinically manages patient movement between units from the PACU, Emergency Department, outpatient clinics, procedural clinics and pre-admits for all service lines. Breast Oncology Nurse Navigator

Overview About the Role The Oncology Nurse Navigator (ONN) works with cancer patients and their families to remove barriers to care, provide emotional support and resolve issues related to the cycle of cancer care. The ONN works as a liaison to and in collaboration with the patient, physician, and other healthcare providers to manage the complex needs of cancer patients. She/he serves as an advocate for the patient and works to enhance the patient experience, breaking down barriers so the patient receives timely and compassionate care. The ONN is involved in community outreach and education and serves as an expert in their clinical area of focus. She/he will serve as a tumor board coordinator for their assigned disease focus and provide clinical care as directed for their assigned patient population. Additionally, the ONN is proficient in quality practices and will track navigation metrics and quality indicators for reporting to Cancer Committee and for accreditation purposes. Prepare reports for the Cancer Committee and assist with accreditation and regulatory requirements. Promote evidence-based practices and continuous quality improvement in cancer care delivery. Qualifications What You’ll Bring Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred. Minimum of four (4) years of clinical nursing experience required, preferably in oncology and/or the assigned disease specialty. Current New Jersey Registered Nurse (RN) license required. Oncology Nursing Certification required or must be obtained within 24 months of employment and maintained throughout employment. Knowledge of the healthcare system, cancer care, community resources, and social service resources for cancer patients required. Strong leadership, organizational, written, verbal, and interpersonal communication skills required. Ability to work independently in a self-directed environment and effectively lead and collaborate with teams. Demonstrated empathy and a service-oriented approach when engaging with patients, families, and community groups. Ability to exercise independent judgment, tact, and diplomacy in all situations. Proficiency with Microsoft Office Suite required. Ability to work flexible hours. Bilingual language proficiency preferred. Overview About the Role The primary purpose of the Staff Nurse position is to provide direct patient care, perform nursing procedures, assess, plan, implement, and evaluate care for diagnostic and interventional cardiology patients in a Cardiac Catheterization Lab setting as observed by the Nurse Manager/Assistant Nurse Manager, and patient/family feedback unless otherwise noted.
