The System Patient Logistics Nurse Coordinator has the responsibility of coordinating the movement of patients throughout the entire Premier Health System and outside transfers into Premier Health. The nurse will utilize her/ or his clinical expertise and critical thinking skills to ensure a seamless transfer process for both Internal and External customers. The nurse will gather medical information pertinent to the level of care which will allow her to guide the referring and accepting physician to make the best choice for appropriate level of care for the patient. The nurses’ understanding of the ‘comprehensive plan of care’ will ensure that the patient is placed on the level of care necessary to ensure delivery of quality safe care and provide a positive patient experience. The Patient Logistics nurse will be cross trained in Central Logic application to facilitate rapid bed placement, monitor bed placement by utilizing the EPIC bed boards for the system, work with physicians, nursing leadership, and EVS to expedite discharge of patients. These functions are designed to decrease length of stay, and decrease patients leaving without being seen, ultimately increasing patient satisfaction/experience.
