Location: Parkland: 16th FLR BLK 400 **NIGHT SHIFT** Full Time Position Primary Purpose A Registered Nurse (RN) with > 2 years of experience. The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, that focuses on a particular patient population(s)/specific health conditions, by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and the healthcare environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition and the competencies of the employee. Minimum Specifications Education Must be a graduate from an accredited school of nursing. BSN, preferred Experience Must have two years of nursing experience. Prefer experience in an acute care setting. Must be able to manage an adult ICU patient with neurological, pulmonary, cardiovascular, gastrointestinal, genitourinary disorders/conditions. Must provide safe medical administration using the patient's six (6) rights, provide maintenance and management of IV access, must be able to perform POC monitoring, demonstrate safe infection control practices and CRRT for the adult ICU patient. Supports pain management as appropriate for each individual patient, provides emergency resuscitation care of an adult and provides an individualized plan of care. Maintains and implements knowledge of applicable rules, regulations, policies, laws and guidelines that impact or govern nursing practice, and ensures the hospital and Parkland are in compliance. Develops effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Maintains and updates knowledge of sanitation, infection control, safety, supplies, equipment, appropriate utilization of supplies and materials. Completes appropriate documentation. Maintains accurate and timely entries and notations in patient care records and hospital documents. Ensures that all records and documents meet the standards, goals, and objectives of the Adult ICU/Critical Care department and Parkland. Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Graduate of an approved Registered Nurse program and licensed in the state of practice, required. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. • Knowledge of the care planning process. • Experience with MDS 3.0, preferred. Responsibilities Transitions patients through all of the phases of the pre/post-transplant process including pre-transplant (referral, evaluation, listing), peri-transplant (inpatient rounding, discharge planning, and education), and post-transplant follow-up. Directs liaison activities to appropriately integrate the patient into the health care continuum including procuring services, health promotion and counseling, disease prevention, health education and screening, and community resource linkage. Assists physicians with exam, provides consultation on patient progress, locates lab results, charts or other needed data and schedules follow-up appointments to ensure that patient visits are efficient and effective for both staff and patient. Triages telephone calls, refers patients to emergency room or schedules appointments. Evaluates and educates patients regarding the signs and symptoms of rejection, explains hospital policies and procedures to ensure that all telephone communications promote positive health practices and outcomes. Implements discharge plan for patient to ensure coordinated care and optimal outcomes. Reviews physician treatment plan and educates patient regarding medications, appointments and lab work. Coordinates home health care concerns by contacting physicians and obtaining orders or clarification and communicating back to the agency. Completes necessary UNOS and other forms, obtains data from out of state and other sources to ensure that complete and accurate records are maintained. Maintains a positive working relationship with management, vendors, employees, other hospitals and physicians, dialysis units and centers, lab and nursing staff to ensure optimal cooperation and communication with all patient service providers. Coordinates referrals to outside nephrologists. Discusses case with nurse, obtains necessary patient release form, explains process to patient. Follows up on any missed appointments to ensure patient compliance. Identifies and analyzes the design of jobs, work processes and flows for the Transplant OPC department and suggests or implements appropriate changes to improve effectiveness, productivity and efficiency that support the overall goals of Parkland.
