Responsibilities We are actively hiring for an Registered Nurse (RN) PreOp PACU. Promote the mission, vision, and values of SCA . Assess patient’s preoperative and postoperative status at intervals according to facility or health care organization policies and procedures to include the patient’s cardiovascular system, respiratory system and airway management needs, neurological system, hemodynamic stability, pain and comfort needs, and mental status. Interpret information accurately from various noninvasive and invasive monitors. Describe the underlying pathophysiology of commonly encountered disease processes. Describe the physiology associated with various anesthesia techniques and surgical or procedural interventions. Apply the relevant scoring system/discharge criteria for patient transfer ( e.g. , Aldrete Score, Modified Aldrete Score [ i.e. , PARSAP], Post Anesthesia Discharge Scoring System [PADSS], White) score accurately as defined by organizational policy. Develop an individualized perianesthesia plan of care. Implement the plan of care including the following interventions: Preparing for and managing patient recovery from anesthesia – general. Preparing for and managing patient recovery from anesthesia – monitored anesthesia care. Preparing for and managing patient recovery from anesthesia – regional. Preparing for and managing patient recovery from anesthesia – local. Preparing for and protecting patient from injury caused by positioning, thermal sources, and extraneous objects. C ommunicating the patient’s current status throughout the perianesthesia continuum of care. P roviding acute pain and comfort management as needed. A dministering medications safely and correctly by using the 8 -rights of safe medication administration. P erforming interventions to maintain the integrity of the patient’s wound and tissue perfusion at or above baseline levels . P erforming interventions to maintain the status of the patient’s genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, and neurological systems, and fluid, electrolyte, and acid-base balances at or above baseline levels. P erforming interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period. P erforming interventions to protect patient from infection. M anaging patient’s other co-morbid conditions as applicable. M anaging patient’s fluid as prescribed. A ssessing knowledge level of patient or designated support person and providing education regarding the expected psychosocial response, nutritional management, medication management, pain and comfort management, wound management, and expected responses to the operative or invasive procedure; I nvolving patient or designated support person actively in decisions affecting his or her perioperative plan of care and the rehabilitation process. P rotecting patient’s rights, dignity, and privacy. P roviding age-specific, culturally competent, ethical care within legal standards of practice; and P roviding consistent and comparable care regardless of the setting. Evaluate the patient’s progress toward attaining outcomes. Transfer care as needed. Provide hand-off report to other teammates that are accurate, concise and pertinent to the patient’s condition and response to care. Delegate tasks according to the state Nurse Practice Act (if applicable), state board of nursing or professional registration requirements, state laws, and job descriptions. Document nursing care completely, accurately, and legibly. Collaborate effectively with other disciplines as needed. Participate in quality review and performance improvement projects. Successful completion of Advanced Cardiovascular Life Support (ACLS) Course within 90 days of employment or documentation of current ACLS certification. Facilitate patient/family group education sessions. Coordinate patient flow and triage patients in the absence of the provider when provider is out or unavailable. Update on-call providers daily on changing needs of complex, high maintenance or high-risk patients. Maintain infection control within the clinic. Perform laboratory procedures when necessary. · Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). · Valid driver's license, reliable transportation, safe driving record and insurance coverage required. · Perform other duties as assigned. This position will be based out of the VG 7th Ave clinic and will require travel to the RCP program site in Hillsboro and may require travel to local hospitals for discharge planning. Essential Duties and Responsibilities: · Screen referrals from hospitals and coordinate with discharge planners to assess patient acuity, program requirements, and bed availability to determine appropriateness for RCP admission. · Identify potential clinical and supportive service needs while within respite program. · Proactively communicate with RCP participant’s primary care team (VG or external) to facilitate early post-hospital contact. · Obtain required consents and releases for coordination of care. · Develop care plan based on patient goals and the reason for admission to RCP. · Gather information for a clinical care plan including physical and psychological history and assessment; list of current medications, dosages, and allergies; vital signs; other pertinent information, such as laboratory or other test results. · Identify discharge indicators and develop a discharge plan with patient. · Participate in and facilitate multi-disciplinary care coordination meetings with appropriate providers (e.g., PCP, home health, pharmacies, specialty care). · Coordinate with shelter case management to address SDOH and other non-clinical needs, · Provide shelter staff education and support around basic medical knowledge as relevant for evening and weekend supervision of RCP residents. · Huddle daily with RCP team to create and implement daily action plans, performing transition of care visits for patients being discharged from hospitals, following up with outside provider recommendations after outpatient visits and ensuring that treatment recommendations and medication changes are acknowledged and actualized. · Facilitate patient access to obtain durable medical equipment (DME), wound care, oxygen, and incontinence supplies (as needed). · Provide patient education and health coaching on topics including diabetes, hypertension, and other chronic diseases, including assisting patients with self-management goal setting. · Educate patients on how to utilize after-hours number or daytime nurse triage to address immediate clinical needs and provide guidelines for when to contact RCP team or PCP vs. seeking emergency department care. · Log vital signs and monitor for changes in health status as indicated by the care plan. · Administer pre- and post-procedural or operative care, including minor wound care per discharge instructions, monitor for changes in health, and recommend follow-up services as needed. · Support self-administration of medications and patient progress towards self-management of medications. General Role Description: The Home Health Patient Care Manager/RN is responsible for the supervision and coordination of clinical services and provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations. Follows-up on lab and other clinical diagnostic test, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow-up, and needed plan of care modifications and communicates such clinicians. Schedules, prepares for, facilitates, and documents case conference/SOC reports and facilitates effective exchange of information across disciplines especially with adverse findings, changes in patient condition, daily and urgent updates, as necessary. Assists clinicians in coordinating the transfer and discharge of patients from agency services as indicated by the physician. Receives report from field clinicians prior to scheduled days off on patient status and ongoing needs. Follows-up with On-Call events daily Assures payer change documentation is completed properly and timely, and required. Reviews clinician visit notes weekly to ensure timely, complete, appropriate, and accurate submission of all documentation by field staff. Takes necessary action to correct adverse findings and communicates trending to clinical director. Reviews, evaluates, and supervises service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies through activities such as random patient visits, medical record reviews and case conferences. Enters infections and incidents/occurrences into the online Risk Management Incident Reporting System, as specified by policy. Assists in the orientation of new agency personnel and provides direction and leadership to clinical team members in collaboration with the clinical director. Job Summary: The Call Center Advice Nurse (CCAN) provides clinical advice and support over the phone to patients calling our call center.
