PRIMARY DUTIES AND RESPONSIBILITIES: A. Delivers expert, evidence-based nursing care to patients and families. 1. Implements nursing process:  Assess veins and access needs and makes recommendations to providers accordingly.  Assesses patient and family needs.  Evaluates home line care routine and supports patient to achieve optimal outcomes.  Analyzes assessment data to develop and support clinically accurate diagnoses.  Plans care by establishing goals with measurable outcomes.  Implements the plan of care.  Evaluates the patient's response to interventions. 2. Documents care according to Institute and regulatory agencies' requirements. 3. Administers medications relevant for care of the venous access patient and/or specific to Imaging protocols and performs procedures safely and efficiently in accordance with Institute policy. May provide specialized care of protocol drug patients in various settings. 4. Evaluates educational and psychological needs of patient and family and incorporates identified needs into the plan of care. 5. Educates patient and family regarding various aspects and treatment modalities of disease, including vein choice, vein preservation techniques, and line care if applicable. 6. The OneCare program is a state initiative to promote care coordination, continuity of care, and improved health outcomes for those clients under 65 who are eligible for both Medicare and Medicaid. In this role, you will mostly focus on performing Uniform Core Assessment (UCA) assessments and creating care plans customized to meet the needs of individual clients. UCA training is provided by Commonwealth Care Alliance (CCA)/CareSource. You will also help update these plans both annually and when a client has had a significant change in status. You will be required to provide face-to-face outreach to meet with clients, assess their status, and support the client’s progress toward better health outcomes. You will also connect with clients by phone, communicate with providers about client needs, and connect clients to eligible services including visiting nursing, nutrition, durable medical equipment, and social services. Clients may be housed; living in shelters or on the streets and you will need to be resourceful to connect with our clients. It is anticipated that success with connecting with clients will require communication with BHCHP care teams and shelter partners, use of BHCHP and CCA/CareSource electronic medical records, as well as outreach to clients who are living on the street, shelter, or in temporary or permanent housing. You will need to remain flexible and creative, as this role will continue to evolve as experience is gained. There is potential for this role to expand to full-time hours in the future if the candidate is interested. Maintains awareness of MSDS sheets and how to access them. c - Assesses each patient's risk for falling throughout hospitalization using appropriate interventions to prevent falls. d - Ensures environment meets regulatory requirements at all times (ie: no cardboard boxes on floor, equipment cords are secured and plugged into foot of bed, etc...). e - Understands and is able to demonstrate individual roles and responsibilities in teh event of hospital codes/emergency preparedness. f - Practices patient hand off utilizing the principles of SBAR - including walking rounds. 7 - PROFESSIONAL DEVELOPMENT Assumes overall responsibility for own professional development by incorporating evidenced-based practice, research, and performance improvement initiatives as a part of ongoing nursing practice. a - Obtains at least 5 contact hours per year in area of practice. b - Practices within the legal boundaries of MA Nurse Practice Act.
