Role Overview As a Home Health RN, you are the clinical lead for your patients - managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge. You’ll combine advanced assessment skills, care planning, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home. Job Purpose The RN Shift Supervisor - Complex Care is the first line supervisor on a nursing unit that practices and directs patient care activities during assigned shift.
