Help patients and families understand and navigate topics such as: Medication administration Hospice philosophy and services Symptom and pain management End-of-life processes and expectations Essential Functions Job Responsibilities Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments/screenings and provide education to patients, family members and caregivers as appropriate. Deliver skilled nursing interventions and symptom management aligned with goals of care. Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification. Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners. Provide patient and caregiver education regarding disease process, self-care techniques, end-of-life care, medications, nutrition and dietary needs. Communicate change in condition, or signs and symptoms of decline to appropriate provider (e.g., NP, PCP, Hospice Physician). Support the continuity of care by recognizing and supporting home health, palliative and hospice referrals as appropriate and recognizing and supporting transitions in levels of care. Document timely clinical records on all patients including screening/assessment findings, physician orders, progress notes, and care plans. Maintain effective working relationships with IDG, and involve team when patient's care plan warrants team participation to achieve desired outcome. Participate in on-call and patient support activities as applicable, conducting on-call services in a clinically competent and responsive manner. BASIC PURPOSE Oversees primary caseload of Hospice patients and provides skilled professional nursing care to them, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Michigan Hospice licensure laws, accrediting body standards and agency policies and procedures. B. PRIMARY RESPONSIBILITIES Nursing Process As a member of the IDT, the Registered Nurse (RN) utilizes the nursing process to contribute to each patient’s total plan of care. Assessing and Planning 1. Seek and obtain sufficient information necessary to function effectively in the Hospice setting where the patient/family/caregiver is receiving care. 2. Conduct initial assessment, including physical, emotional and psychosocial aspects, of patient/family/caregiver in a thorough, accurate, compassionate and timely fashion. 3. Utilize the nursing body of knowledge to provide leadership and guidance to the IDT in formulating and individualizing the interdisciplinary plan of care to include problem identification, interventions and goals to address palliative comfort measures for the terminally ill patient, family and caregiver. 4. Contribute to identifying both active and potential needs of the patient and his/her family, which may affect the plan of care. 5. Consistently collaborate with the physician to plan and implement the medical and nursing plan of care. 6. Review the nursing component of the comprehensive interdisciplinary plan of care with the IDT. Communicate in writing in the clinical record and verbally with the IDT to coordinate interventions and ensure continuity of care. 7. Facilitate and participate in the IDT care conference and contribute to patient care planning. Implementing and Evaluating Provide nursing interventions and appropriate preventative and therapeutic nursing procedures. Nursing staff shall administer medications in accordance with all Federal and State laws and rules. Demonstrates expertise in the utilization of all IDT members including volunteers and community resources. Fulfills productivity requirements on an average daily census basis as determined by acuity level and Administrator. Documents accurate, complete and relevant clinical notes regarding patient’s condition and care given. Maintains documentation a on a timely basis. Assesses the dietary/nutritional needs of Hospice patient and provides appropriate interventions relative to nutritional intake as the patient’s condition progresses. Education The RN is a mentor who is able to provide educational experiences and communicate knowledge of patient care to members of the IDT including, but not limited to, volunteers, hospice aide/homemaker, patient and family/caregivers, health care professionals and the general community. Acts as a mentor to other personnel by participating in orientation and assisting with field training and in-service education to contractors and referral sources, as needed. JOB SUMMARY A professional who provides professional nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates patients and their families. Professional skilled nursing is performed under the supervision of the Administrator/Director of Nursing. QUALIFICATIONS Graduate of an approved professional nursing program (RN) Currently, licensed “in good standing” in MI Minimum of one (1) year of experience as a professional nurse, preferably in home care. Must have a criminal background check. Must have current CPR certification. RESPONSIBILITIES/ESSENTIAL FUNCTIONS Furnishes those services requiring substantial and specialized nursing skill. Initiates appropriate preventative and rehab nursing procedures. Prepares & submits timely, clinical and progress notes. Oversees coordination of patient services. Informs MD and other staff of changes in PT situation or needs. Completes medical history/home safety check/environmental assessments. Performs socio-psychological evaluation. Performs assessment visit and documents timely. (ie. OASIS/ Skilled Nursing Note etc.). Performs physical examination and review of all body systems and documents accordingly. Develops individualized Plan of Care (POC) to be submitted to the physician for approval and implementation and performs necessary revisions based upon patient needs. Determines medical necessity for other services. Regularly re-evaluates the patient’s nursing needs. Evaluates the patient’s ADL and iADL abilities and need for home health aide. Develops and implements the HHA plan of care when HHA services are ordered. Revises and signs this care plan the beginning of each certification period. Supervises HHA in accordance with state/federal requirements and documents the supervision. Orders “other” professional services that may be appropriate to the needs. Reviews billing processes with patient and/or family advising patient and/or family when co-pay or Medicare is not likely to pay for services. Effectively communicates ongoing with patient and family progression/changes in POC. Effectively communicates with Supervisor/Nursing Director/other disciplines in the case. Communication with the patient’s MD (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities. Actively communicates in the case conferencing sessions to establish best practices. Submits requests for re-authorization of “more visits needed” to payers timely. Coordinates Community Services for the patient to assist in safe home care needs. Participates in the Performance Improvement Committee process. Performs timely clinical record reviews (CRR) per Agency policy in collaboration with the Nursing Supervisor. Participates in ongoing staff meetings/in-services. Participates in the growth of office by being a willing preceptor for same discipline employees. Participates in the planning, operation and evaluation of the nursing services of the organization. Maintains professional licensure per state requirements. Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event’s occurrence. Submits written time requests 2 weeks or more in advance of planned time off. Other duties as assigned. SPECIAL EQUIPMENT TO BE OPERATED Vehicle to do routine patient visits, possible transfer/lifting devices, medical supplies WORK ENVIRONMENT Indoors, performing patient care assessment/tasks in the patient’s residence Travel to patient residences/meetings; will require transportation to get to various work sites Possible exposure to communicable diseases FUNCTIONAL ABILITIES Able to access patient’s homes that may not be handicap accessible. Able to hear, speak, understand and communicate effectively in English. PHYSICAL REQUIREMENTS On a daily basis: walking, standing, sitting, reaching, use of telephone, use of computers & other office equipment, lifting/transferring 150 lbs or greater, bending, stooping, climbing stairs.
