This senior leadership role is responsible for overseeing health services, wellness programs, and ensuring regulatory compliance in line with New York State Department of Health (DOH) standards. The DON plays a vital role in creating a safe, resident-centered environment that supports dignity, quality outcomes, and excellence in care. Key Responsibilities: Oversee all clinical care and health-related services within the Adult Home (AH), Assisted Living Residence (ALR), Enriched Housing Program (EHP), Enhanced Assisted Living Residence (EALR), and Special Needs Assisted Living Residence (SNALR). Ensure full compliance with NYS DOH regulations and internal policies. Develop and implement individualized service plans (ISPs) in collaboration with Case Managers and the Administrator/EHP Coordinator. Lead and evaluate nursing staff; supervise Home Health Aides (HHAs) assigned to medication management and personal care. Conduct initial and periodic resident assessments and reassessments (at least every 6 months or as needed). Maintain accurate clinical documentation, statistical data, and quality assurance reports. Participate in the development, evaluation, and revision of clinical policies and procedures. Serve as a liaison between residents, families, interdisciplinary teams, and external healthcare providers. Manage emergencies and clinical interventions across all care units.
