DaVita Kidney Care is bringing our industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have experience in dialysis, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to Get Stuff Done (GSD). Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the organization. What you can expect: In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. DSD RNs will troubleshoot, and make sound decisions for our patients related to their dialysis treatment and will occasionally partner alongside the SNF staff to coordinate care. Primary work schedule will be 3-4 days a week with some travel to other SNF clinics within your region (depending on business and patient needs). Alternating Saturdays will also be required. As a DSD Services RN, you will be provided the opportunity to conduct impactful and meaningful education to patients and their families in the SNF setting. Direct Patient Care: The RN’s primary responsibility is to independently demonstrate best practice techniques and deliver optimal dialysis care. The role includes supervising Dialysis Technician who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program. Complexity - DSD Dialysis care is a complex but fulfilling service line. Meeting and/or exceeding clinical outcomes are the goals and multitasking is an expectation to meet the needs of the service, the patients and our SNF partners. Autonomy and Critical Thinking. Provide comprehensive patient care in an independent setting. Plan to be on your toes at all times. You use your mind constantly. You apply your "core" nursing skills to assess, diagnose and implement complete care plans as you look at the whole patient. QUALIFICATIONS High school diploma required plus graduation from a formal MA, LPN or RN training program and 2+ years of experience providing clinical care, ideally in primary care. National or state-specific MA, LPN or RN certification (dependent on local regulations). Must be able to give injections, complete EKGs, and perform other office-based procedures. Job Description Summary The QAPI and Regulatory Compliance RN is responsible for QAPI process for assigned agencies. The QAPI and Regulatory Compliance RN is responsible for ensuring that assigned offices are complying with the Community Health Accreditation Partner (CHAP) standards, Center for Medicare and Medicaid Services (CMS) conditions of participation and other regulatory licensures requirements. Essential Job Functions/Responsibilities 1. Review documentation to identify areas which may have fallen out of compliance and communicates findings and recommendations to Clinical Manager and hospice personnel. 2. Tracks and trends information for QAPI (Quality Assurance Performance Improvement) 3. Develops Plan of Correction and/or PIP (performance improvement plan) to bring area to compliance. 4. Performs chart audits as a written into plan of correction and/or PIP. 5. Provides education to bring identified areas into compliance. 6. Visits offices as directed to observe for regulatory compliance. 7. Assists Regional Director of Compliance and Survey Readiness/Compliance Officer to identify areas of risk and develop plan to bring into compliance. 8. Provides education about the QAPI program. 9. Prepares and leads quarterly QAPI meetings for each assigned agency. 10. Assists with policy maintenance and revisions as assigned. 11. Provides education, guidance, and assistance on the Emergency Preparedness Program to agency leadership. 12. Assists with committees, meetings, and team projects relevant to regulatory compliance as well as department/intra-department problem-solving. 13. Assists in the orientation of new employees. 14. Demonstrates understanding of HIPAA requirements with respect to contract reviews, Information related policies and operations for the organization. 15. Assists Regional Director of Clinical Operations and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs. 16. Assists agency leadership which may include, but not limited to, preparing for survey, staff orientation and education, assisting with office clinical operations, conducting patient visits, assisting in covering On Call, as requested. 17. Complies with accepted professional standards and principles. 18. Assesses learning needs of all hospice personnel. Plans and provides education in conjunction with Regional Director of Clinical Operations and other hospice leadership. 19. Assists in developing and writing processes. 20. Assists on other projects and committees. 21. Travel to various assigned agencies. The amount of travel and length of travel will vary by agency need. 22. Performs other duties and activities as delegated by the Regional Director of Compliance and Survey Readiness/Compliance Officer.
