This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. Schedule Full-Time Monday–Friday 8:30 AM – 4:30 PM No nights or weekends! Position Summary As a Hospice RN Case Manager , you will coordinate and manage hospice care for patients in their homes, assisted living communities, and skilled nursing facilities throughout the Scranton/Honesdale territory. Working as part of an interdisciplinary team, you will develop individualized care plans that promote comfort, dignity, and quality of life while supporting patients and their loved ones throughout their hospice journey. Responsibilities Complete comprehensive hospice admission assessments for patients and families. Develop, implement, and regularly update individualized hospice care plans. Perform ongoing nursing assessments and monitor changes in patient condition. Coordinate care with physicians, hospice aides, LPNs, social workers, chaplains, and other members of the interdisciplinary team. Educate and support patients and families regarding hospice services, symptom management, and end-of-life care. Initiate appropriate nursing interventions to maximize patient comfort. Communicate changes in patient condition to the attending physician and hospice medical director as appropriate. Supervise Hospice Aides and Licensed Practical Nurses (LPNs). Maintain accurate, timely, and compliant clinical documentation. Participate in patient visits, case management, and team coverage as needed. Qualifications Current Pennsylvania Registered Nurse (RN) license in good standing. Hospice nursing experience preferred; home health or palliative care experience is a plus. Strong clinical assessment, communication, and organizational skills. Reliable transportation, valid driver's license, and current automobile insurance. Ability and willingness to travel throughout the Scranton/Honesdale service area.
