Job Description Performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. Includes every other weekend and Holidays, plus on-call required on weekdays, weekends, off shift and holidays. Shift can start as early as 6:00pm and as late as 9:00pm. Shift can end as early as 6:30am and as late as 9:30am. * Assesses the patient's physiologic health status Conducts patient interview, explains policies and procedures to patient/significant others, reviews patient's chart and answers questions correctly and courteously; dosage; side effects and implications for the patient; Meets standard on medication administration examination; Demonstrates preparation of local solutions; Demonstrates preparation of I.V drips; Administers and documents medication correctly; Administers IVP medication as per department policy and procedure; Educates patients and significant others related to drug and food interactions. * Participates in patient and family education Provides the patient with an explanation of his/her condition as indicated; Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching; Provides for early discharge planning and appropriate referrals for post-hospital care; Evaluates the effectiveness of teaching by feedback from patient/family and documents same. Job description: We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. Key Responsibilities: Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines. Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records. Conduct health assessments and respond to medical emergencies as needed.
