About the Role We are seeking a compassionate and dependable Registered Nurse (RN) – Evening Hospice Care to join our hospice team full-time. This position is dedicated to providing evening hospice care and support , ensuring patients and families have access to clinical expertise during some of the most critical hours of the day. The schedule may consist of either 8-hour or 10-hour evening shifts and includes working every other weekend . In this role, you will respond to patient and family needs, complete nursing assessments, provide direct hospice care, and collaborate with the interdisciplinary team to ensure continuity of care. Provide visits to address urgent concerns, symptom changes, or end-of-life needs. Deliver skilled nursing care in accordance with patient goals and physician orders. Assess and evaluate patient conditions and provide clinical oversight when LPN support is involved. Develop, update, and implement individualized plans of care. Administer medications and treatments as prescribed. Support symptom management and patient comfort. Educate patients and families on disease progression, symptom management, and care techniques. Provide emotional support and reassurance during difficult situations. Communicate significant changes in condition to physicians and obtain necessary orders. Complete timely and accurate clinical documentation. Coordinate equipment, supplies, and services needed to support quality care in the home. Collaborate with the interdisciplinary team to ensure continuity of care. Assist with admissions, crisis visits, death visits, and other after-hours hospice needs as appropriate. Participate in continuing education, quality improvement activities, and team meetings. What You'll Bring Current Virginia Registered Nurse (RN) license. Graduate of an accredited nursing program; BSN preferred. Two or more years of hospice, home health, ICU, critical care, or related nursing experience preferred. CHPN certification preferred. Current CPR certification. Strong assessment, critical thinking, and clinical decision-making skills. Knowledge of Medicare and Medicaid hospice regulations. Proficiency with electronic medical records. Ability to travel throughout the Blue Ridge Care territory. A calm, confident presence during urgent and emotionally challenging situations. As a part of the VCU Massey Cancer Center, the largest adult stem cell and bone marrow transplant provider in all of Virginia, we offer the latest FDA- approved CAR-T immunotherapies, clinical trials testing new and promising forms of cellular immunotherapies, as well as all forms of stem cell and bone marrow transplantation. We provide both outpatient and inpatient services in the same facility, and provide a suite of support services that range from helping minimize symptoms and side effects to coping with the cognitive, emotional, and psychological effects of cancer. A typical shift for our nurse includes a team of mixed registered nurses, clinical coordinators, and Patient Care Techs (PCTs) caring for adult patients. Acute Care Pediatrics 12 is a 24-bed pediatric unit. The staff care for patients from a variety of medical specialties including, but not limited to nephrology, neurology, cardiology, pulmonology, gastrologic, and endocrinology. The unit also sees surgical specialties such as trauma, plastics, and general care surgery patients. The unit consists of a combination of general care and step-down patients. Special to ACP 12 are the hematology/oncology, epilepsy monitoring and bone marrow transplant populations. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed.
