HOW YOU'LL WORK: You'll provide skilled nursing care in a patient's home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you don’t already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change. Minimize Patient Risk: Administers medication as prescribed by the physician and helps decrease re-hospitalizations by prioritizing visits for high-risk patients. Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Educate the patient and family on the disease processes using teach back methods to ensure patient and family understanding. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborates with the interdisciplinary team and physicians to ensure optimal care is provided. Rules and Regulations: understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. This 32‑bed unit supports recovery after complex cardiovascular surgeries, including open‑heart procedures, heart failure interventions, and advanced device therapies. Nurses in this unit work closely with cardiothoracic surgeons, intensivists, NPs, and a full multidisciplinary team to deliver 24/7, high‑acuity care. The CVICU offers hands‑on experience with ECMO circuits, Impella, LVADs, balloon pumps, and other advanced life‑support technologies — with educational collaboration through the Cleveland Clinic. Job Description Interview’s patients to determine medical problem/condition and documents in chart (EMR) for physician, when necessary obtains and records patient’s vital signs including weight and assists physician with patient examination as needed and explains procedures and treatments to patient to gain cooperation, understanding and allay apprehension, maintains awareness of comfort and safety needs, manages lipids, protimes and phone triage of patient concerns. Documents nursing history in EMR and physical assessment for assigned patients and initiates a patient education plan according to the individualized needs of the patient, as prescribed by the physician. Abstracts patient records via the EMR. May sort or do chart prep on occasion but not as a routine part of the position. Uses other EMR functions such as tasking, messaging, and disease management.
