What You'll Do as a Hospice RN / RN Case Manager: Be the primary point of contact for patients and families, guiding them through every step of their hospice journey. Maintain accurate, up-to-date clinical documentation and follow infection control and compliance standards. Support the organization's quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient. Help patients and families understand and navigate topics such as: Medication administration Hospice philosophy and services Symptom and pain management End-of-life processes and expectations Essential Functions Job Responsibilities Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments/screenings and provide education to patients, family members and caregivers as appropriate. Deliver skilled nursing interventions and symptom management aligned with goals of care. Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification. Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners. Provide patient and caregiver education regarding disease process, self-care techniques, end-of-life care, medications, nutrition and dietary needs. Communicate change in condition, or signs and symptoms of decline to appropriate provider (e.g., NP, PCP, Hospice Physician). Support the continuity of care by recognizing and supporting home health, palliative and hospice referrals as appropriate and recognizing and supporting transitions in levels of care. Document timely clinical records on all patients including screening/assessment findings, physician orders, progress notes, and care plans. Maintain effective working relationships with IDG, and involve team when patient's care plan warrants team participation to achieve desired outcome. Participate in on-call and patient support activities as applicable, conducting on-call services in a clinically competent and responsive manner. Summary The Clinical Nurse Care Manager is responsible and accountable for coordinating the care of a specified group of patients, specifically Medicare patients conducting annual wellness visits. Assesses, plans, implements, coordinates, monitors, and evaluates options to facilitate the continuum of care. Assess patient needs for healthcare resources based on medical condition, patient resources, and potential barriers to optimal progression through the healthcare system. Functions as a resource to clinical staff regarding patient assessment and plan of care or performs patient assessment and develops a plan of care to assure consistent and appropriate care is provided in a patient focused manner. Facilitates discharge planning efforts through early identification of high risk patients. Manages each patient's transition through the system and transfers accountability to the appropriate person upon entry into another clinical service. Serves as a patient advocate to assure patient education is completed. Plans and implements (through multi-disciplinary meetings, messaging and conversations with the care team, and with patient or family) strategies to reduce resource consumption, and achieve high quality positive patient outcomes. Evaluates and measures outcomes in order to continuously improve practice and to enhance outcomes. Identifies changes in patients’ clinical progression or needs to assure continuity of care. The following may be assessed to determine needs: medical, rehabilitative, psychosocial and physical needs. Assures timely referral to other services to facilitate ongoing care needs of patients. Serves as a patient advocate. Enhances a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions. Coordinates the provision of social and other support services to patients, families, and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from healthcare services. Utilizes appropriate resources in cases that present ethical dilemmas. Communicates with payers to facilitate the patient’s effective and efficient movement through the healthcare system. Understands and adheres to the college Medical Center compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy. Keeps abreast of all pertinent federal, state, and the college's regulations, laws, and policies as they presently exist and as they change or are modified. Ensures that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs. Assists with quality gap closures utilizing various BCM approved platforms. Conducts patient outreach to schedule visits for patients. Performs other job related duties as assigned. Minimum Qualifications Bachelor's degree in Nursing. Four years of relevant experience. Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing. Current Basic LIfe Support certification from the American Heart Association. Preferred Qualifications Eligibility or Certification in Case Management by the Texas Department of State Health Services (DSHS). Ten years of nursing experience.
