Management of patients having or requiring: General post-surgical management Bariatric surgery Sepsis Pneumonia COPD management Diabetic management Osteomyelitis Pancreatitis The Medical/Surgical unit at Piedmont Eastside uses: Bedside report handoff at change of shift Purposeful Rounding for safety and service Quality initiatives such as pain assessment and reassessment, pressure ulcer prevention, infection prevention strategies for central lines and indwelling urinary catheters, etc. Unit educator to help address competency, onboarding, and learning needs Charge nurse out of staffing or with limited patient load to facilitate daily operations EPIC computer system Responsibilities: RESPONSIBLE FOR: The staff nurse provides nursing care to patients from birth through the lifecycle utilizing nursing processes to assess, plan, implement, and evaluate the care for patients. He/she functions within the framework of the policies and procedures of the organization and demonstrates professional growth and accountability. The staff nurse is responsible for maintaining standards of practice, coordinating patient care activities of all assigned staff in the provision of quality nursing care. ESSENTIAL FUNCTIONS: 1. Performs patient assessments to include collection of data regarding the patients physical, psychosocial and health history. 2. Appropriately evaluates the developmental age of the patient when performing assessments, treatments and patient/family education. 3. Organizes patient data into the formulation of a plan of care; identifies additional information needed and makes care decisions based on identified needs. Identifies expected patient outcomes. 4. About the Role: The Women & Children Resource Pool consists of 3 subset teams known as “Pods” (Labor & Delivery Pod, NICU Pod, and Mother Baby/Pediatric/Step Down NICU Pod). The team works within the Women & Children service areas of Northeast Georgia Healthcare System and will report to our Gainesville, Braselton, and/or Demorest (Habersham) locations. Each “pod” is specialized according to patient population and requires 2 years of RN experience specific to specialty of the pod. Labor & Delivery Department’s Overview The Gainesville Labor and Delivery Unit, consisting of 18 labor/delivery/recovery beds, 8 triage beds, 4 operating rooms and 6 PACU bays. The Braselton Labor and Delivery Unit, consisting of 10 labor/delivery/recovery, 2 triage beds, 2 operating rooms and 2 PACU bays. The Habersham LDRP (Labor/Delivery/Recovery/Postpartum) unit consists of 9 labor/delivery/recovery/post-partum beds, 1 triage bed, and a 2 bed well baby nurserys. Assess, coordinate and facilitate patient's discharge plan to assure post-acute needs are arranged and secured prior to discharge; Communicate discharge plan with Physician, patient/family, and other members of the healthcare team as appropriate; Reassess discharge plan routinely throughout patient's stay to ensure timely, safe discharge and appropriate transition to the next level of care. Provides patient/family with information regarding their plan of care, discharge and any financial responsibility of inpatient or post-hospitalization services. Maintain knowledge of reimbursement methodologies and general coverage guidelines for all levels of inpatient and outpatient care. Communicate with Physician, patient/family or other team members as needed to ensure services will be covered. Coordinate and communicate with Utilization Review Nurse on a daily, consistent basis to ensure patients are in the right status and level of care. Facilitate changes by communicating with Physician, mid-level or nursing staff as needed. Serve as liaison to patients family, Physicians, nursing staff and all other disciplines to achieve optimal outcomes in the development of patient's discharge plan. Serve as a leader on assigned unit in the areas of discharge planning, social service issues, community resources/referrals and financial information related to patient care and outcomes. Actively supports a customer service oriented environment to continually enhance customer service; Communicates directly with Physicians, nursing staff, patient/family and other disciplines to ensure collaborative practice. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team.
