The RN Care Manager functions as a principal member of the multidisciplinary care team to facilitate initial and ongoing evaluation of patient and family needs and drive the coordination of care intended to progress the patient safely and efficiently from admission, through the continuum of care, and discharge to home. The RN care manager proactively supports the progression of care collaboratively with the patient, patient representative, medical providers, social workers, nursing staff, therapy services and appropriate members of the care team to facilitate the highest level of patient-centered care while ensuring the appropriate transitional or discharge plan back to their home environment or post-acute setting.
