The RN care manager leverages industry best practices with an emphasis on safety, standardized processes, and analytics tools to build a collaborative care team approach for patients and families. The RN Care Manager functions as a principal member of the multidisciplinary care team to facilitate initial and ongoing evaluation of patient and family needs and drive the coordination of care intended to progress the patient safely and efficiently from admission, through the continuum of care, and discharge to home. The RN care manager proactively supports the progression of care collaboratively with the patient, patient representative, medical providers, social workers, nursing staff, therapy services and appropriate members of the care team to facilitate the highest level of patient-centered care while ensuring the appropriate transitional or discharge plan back to their home environment or post-acute setting. This includes coordination of services provided by community agencies such as home care or providers of medical equipment. Assessment of the patient will include but is not limited to completion of patient discharge assessments, review of medical records to determine complex needs of the patient, length of stay, management, and denials prevention. The Physician Advisor role will support the RN Care Manager for second level clinical reviews for cases that do not meet medical criteria of complex cases. The NNP serves as an expert clinical nurse who utilizes specialized knowledge and skill obtained from advanced formal education and clinical expertise to promote optimum care of patients and families with acute neonatal issues. The NNP assesses the health status of the pediatric client (0-2 years of age) through interview, health history, physical examinations, and diagnostic tests.
