Nurse (RN) – Capacity Coordination Center, Rotation - Per Diem

Nurse, Capacity Coordination Center 12 hour shifts, D/N Rotation - Per Diem status As part of a multi-disciplinary team, applies the nursing process to develop a plan using telephone triage and review of the medical record for patients who require urgent/emergent DH consultation, outpatient procedure or transfer as well as transfers between levels of care during patients’ inpatient hospitalization. Capacity Coordination Center Nurses are responsible for providing a high level of customer service in a high stress, fast paced environment while maintaining flexibility and the ability to adapt, improvise and problem solve in rapidly changing conditions. Responsibilities Obtains and documents relevant clinical data to individually assess the needs of each patient to ensure appropriate level of care and proper placement of patients. Acts as a navigator for referring providers in identifying and accessing appropriate DHH services. Manages and coordinates communications between referring and DH providers via telephone and electronic communication methods. Identifies and facilitates patients for redirect and placement to DHH affiliates. In collaboration with the On Call Medical Director determines appropriateness of requests for outside transfers, and identifies patients who are considered high priority such as EMTALA, active cancer care, dialysis, DH readmission, etc. Obtains and communicates to DH providers, relevant medical records and images from referring providers. Arranges and coordinates local, regional and international transfers to DH, for patients who require tertiary care or who have established care at DH. . Works with medical staff to appropriately re-prioritize transfer queue. Communicates and collaborates with DH medical staff to prioritize activities associated with accurate transfer timing, placement needs, and bed assignment. . Coordinates and actively participates in daily capacity huddles with On-Call Medical Director, administration, and designated departmental charge nurses to develop relevant mitigation strategies that maximize safe patient intake and patient care resource needs. Advocates on patient behalf in obtaining access to appropriate DHH services. Directs, coordinates, and updates patient transfer queue. Coordinates and assists in developing transportation plans for Neonatal, Pediatric, and Adult Critical Care patients. Identifies and delegates appropriate tasks to assistive personnel/coordinators. Develops and promotes long-term relationships with house supervisors, charge nurses, referring facilities and providers. Determines and assigns accurate patient admission type following established guidelines and criteria. Collaborates with other to maximize day to day patient and bed flow management. This includes, but is not limited to, preparing for critical care bed meetings, monitoring, interacting with, and ensuring accuracy of electronic patient placement systems data, providing back up for patient placement discussion and decisions. Implements standardized and consistent patient placement processes across all areas of responsibility. Assigns locations/beds for stepdown and critical care patients. Arranges external, and facilitates internal transfers for stepdown and critical care patients. Implements and communicates patient census procedures. Analyzes, synthesizes, and corrects census errors and act as a resource to ensure accurate census and patient charges. Actively engages in the daily implementation of strategic goals. Engages with leadership to develop, maintain, and improve departmental policies, procedures, and workflows. Interprets and supports policies, procedures, and safety standards to facilitate daily operations. Identifies and defines problem areas, making recommendations for improvement. Bachelor of Science Degree in Nursing (BSN) is strongly preferred. 3 years of Critical Care experience strongly preferred. Leadership experience desirable. Overview Our Behavioral Health Emergency Department unit consists of a 6 beds supporting high risk mental health patients during a critical psychiatric event. Registered Nurse (RN) - Senior Drug Diversion Specialist

Overview Reporting to Director, Drug Diversion Prevention Program, the Senior Drug Diversion Specialist serves as primary liaison to Dartmouth-Hitchcock, the largest and most clinically and operationally complex Dartmouth Health system member. The Senior Drug Diversion Specialist role also supports Dartmouth Health member sites in managing complex diversion investigations and post-investigation cause analysis. In their role, the Senior Drug Diversion Specialist applies, and mentors leaders and peers in, change management techniques, Just Culture principles, systems thinking methods, and data-driven problem solving in the context of diversion prevention, detection, and response. Responsibilities Builds investigation plans for complex diversion investigations carried out across the Dartmouth Health system. Provides oversight and guidance for complex diversion investigations undertaken across the Dartmouth Health system. Leads investigations into suspected drug diversion at Dartmouth-Hitchcock in collaboration with local leaders, drug diversion specialists, and Drug Diversion Rapid Response Team. Leads cause analysis following confirmed or suspected diversion events to gain insight into potential opportunities for system improvement and diversion risk mitigation at Dartmouth Health locations. Provides diversion prevention coaching and consulting to Senior Leaders (Executives), Service Line Leaders/Divisions, and Drug Diversion Specialists. Develops and maintains relationships with system member diversion prevention colleagues through collaboration and communication. Builds and engages in comprehensive surveillance and auditing activities to detect drug diversion, in collaboration with Pharmacy, Nursing and Anesthesia Maintains the structure and processes of a comprehensive Drug Diversion Prevention Program at Dartmouth-Hitchcock. Co-chairs Dartmouth-Hitchcock Controlled Substance Diversion Prevention Program Steering Committee with member location(s) Pharmacy department leader Maintains Dartmouth-Hitchcock drug diversion prevention, detection, and response policies/procedures Provides leadership in multidisciplinary drug diversion prevention education activities Collaborates with all departments/disciplines on controlled substance and drug diversion-related issues, serving as a diversion prevention resource and providing best practice recommendations for controlled substance management Ensures appropriate collection and analysis to drug diversion-related data, and maintains a database containing all data collected that may relate to drug diversion Ensures that internal and external regulatory reporting occurs, as required, following a confirmed diversion event, in collaboration with Office of General Counsel Presents institutional drug diversion reports to Senior Leaders and Board of Trustees as requested Serves as a diversion prevention resource across all Dartmouth Health member locations Performs other duties as required or assigned Qualifications Registered Nurse with 10 years of experience in healthcare safety/quality improvement, regulatory compliance, and leadership. Master’s Degree in Nursing, Analytics, or Healthcare-related field, required. Experience with nursing, pharmacy, hospital, and clinical operations, required. Experience managing drug diversion prevention programs, required. Experience with complex drug diversion investigations, required. Working knowledge of relevant DEA, Board of Pharmacy, FDA, Joint Commission, CMS regulations, required. Excellent interpersonal skills, ability to develop positive relationships with key stakeholders, strong conflict management skills. Ability to work with data and draw inferences from multiple sources and types of information. Ability to analyze complex issues, and develop relevant and realistic improvement plans/recommendations. Experience planning and managing complex projects. Demonstrated excellence in facilitation and teaching. Strong written and verbal communication skills required.
