Position Summary : This is a grant funded position. Registered Nurse Case Manager/Clinical Diabetes Educator is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinic setting. Education is provided via telephonic communication, video teleconferencing, and in-person education sessions. This at times does require village travel. This position will assist in the planning, coordinating and providing of diabetes-related health promotion activities in the YK Delta Region. Educator will work closely with village clinic and hospital staff to promote diabetes standards of care and inform of new advances in diabetes care.
