Location: Sussex Campus Hospital Status: Full Time 72 Hours Shift: Nights SALARY RANGE: 16.98 - 25.48HOURLY General Summary: The Nursing Assistant position involves delivering clinical nursing services to patients. The Nursing Assistant functions under the supervision of a licensed nurse or therapist, providing care that does not require the judgment and skills of a licensed professional. After competency verification, the nursing assistant may deliver patient care services to the infant, pediatric, adolescent, adult, and geriatric populations. The care may include, but is not limited to, the following: bathing, dressing, grooming, toileting, ambulating, transferring, feeding, observing, and reporting the general well-being of the person(s) to whom they are providing care. Coordinates clerical duties of a diverse and confidential nature to maintain the efficiency of patient care areas. Department team members are authorized to have limited access to secured medication rooms to perform duties and responsibilities as defined in this job description. Any staff member requiring limited access to secured medication rooms must have a fully completed and signed Medication Security Agreement for Non-Licensed Personnel form on file and available upon request. Responsibilities: Follows nursing assistant plan of care established by the Registered Nurse within a specified timeframe; including, but not limited to, mouth care, bathing, AM and PM care, assistance with ambulation, transfers to chair and/or bed, taking and documenting vital signs, and recording accurate intake and output. Documents all patient care delivered according to established policies and procedures. Performs duties with the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned units. Maintains a safe, comfortable, and therapeutic environment for patients and families in accordance with hospital standards. Communicate relevant patient information to a professional nurse within an appropriate time based on the level of urgency. Report problems immediately to a professional nurse. Answer intercom or call lights in a timely, courteous manner, with appropriate communication to the patient and/or professional nurse. Performs patient care based on established guidelines for diagnostic and therapeutic exams; i.e., NPO for labs or x-rays, OR, diet changes related to tests, enema-related tests. Perform safety and suicide precaution sitter duties as assigned. Assists other members of the nursing team and/or ancillary departments as requested. All other duties as assigned, within the scope and range of job responsibilities. Required Education, Credential(s) and Experience: Education: High School Diploma or GED, Certificate Program- Must meet one or more of the following criteria: Completion of a Nursing Assistant training course, Medical Assistant training course or Patient Care Technician training course. Completion of a Clinical lab simulation course which included training on vital signs, ADLs etc. Course must be verifiable via official transcript. Active or recent CNA certification in DE or another state. 1 year or more of verifiable work experience in a direct patient care role as a Nursing Asst, Medical Asst, PCT, or Home Health Aide. Credential(s): Basic Life Support Experience: Required: No experience necessary if completed a Nursing Assistant training course, Medical Assistant training course Patient Care Technician training course, or a Clinical lab simulation course which included training on vital signs, ADLs etc. (Course must be verifiable via official transcript.) ~~ No experience necessary if an active or recent CNA certification in DE or another state. ~ If course or certification is not held, one (1) year verifiable work experience in a direct patient care role as a Nursing Asst, Medical Asst, PCT, or Home Health Aide is required. Professional nursing practices, requiring specialized knowledge, judgment, and skills are performed in accordance Bayhealth policies. After competency verification, the Licensed Practical Nurse may deliver patient care services to the adult, and geriatric populations. The care may include, but is not limited to, the following: obtaining and recording in the medical record: blood pressure, pulse, temperature and pulse oximetry, drawing blood, EKG, administration of oral medications, transporting patients, obtain urine and stool specimens, perform point of care testing, assist with procedures consistent with the scope of the employee’s position (i.e.. dressing changes, dressing, grooming, toileting, ambulating, transferring, observing, and reporting the general well-being of the person(s) to whom they are providing care). Responsibilities: The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients. Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing. Provide exceptional customer service that aligns with the Planetree philosophy. Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities. Collects and records intake and output. Prepare and administer oral medications as ordered by the physician or APC. Review medication history with patient. Assist with minor medical/surgical procedures in the practice setting. Performance of venipuncture for the purpose of blood and to monitor the patient’s condition before, during and after the withdrawal of blood. Transport patients using wheelchair or stretcher within offices and to/from vehicles. Assist those patients requiring help with ambulation. Identify the need for patient education and advise patients on their treatment plan. Demonstrate the ability to recognize abnormal lab results. Document treatment records for each patient. Immediately report any observed changes in condition or behavior of patients and documents any care given in an acceptable medical legal fashion. Immediately reports any unusual incidents. Set up for suturing and procedures, using sterile technique; dismantles procedure set, caring for soiled instruments safely and correctly. Maintains competency in specialized testing for Occupational Health (Hearing Booth, Fit testing, Titmus, PFT, Ishahara, Drug screen collections, Breath Alcohol Testing, DS Hair Collections and any other required testing.) Maintain patient confidentiality; comply with HIPAA and compliance guidelines established for the practice. Ensure compliance with Joint Commission regulations and other safety requirements; this includes disposal of used sharps and contaminated materials, as well as, safe storage of medications. Re-order supplies as required and within budgetary limitations. Maintain work station cleanliness and restock linen and supplies to exam rooms; provides general cleaning, including emptying trash, straightening waiting area, etc. Washes and changes stretcher or exam table linen and empties dirty laundry. Demonstrate the ability to reflect knowledge of current professional practice standards, laws and regulations, evaluate their own nursing practice in relation to professional practice standards, relevant statutes and regulations. Engage in an annual self-assessment, identifying areas of strength and areas requiring professional development Seek and reflect on constructive feedback from other members of the healthcare team. Acts to achieve performance goals. Acquire and maintain current knowledge and competency in nursing care. Participate in ongoing educational activities to acquire and validate knowledge (i.e. BLS, NAPPI). Seek experiences that reflect current clinical competencies and new clinical information. Preferred: 4 years ED experience as a Tech / LPN; or 4 years of LPN with medication administration experience.
