Responsibilities: Provide direct care at the patient’s place of residence as indicated. Work collaboratively as a member of the Interdisciplinary Team to assure that the aims of continuity and comprehensiveness of care are accomplished. Understand Hospice philosophy, principles of death / dying. Compassionate communication with their patients and their families. Provides professional nursing care by utilizing all elements of nursing process. Collaborates with the patient and family to promote the physical well-being and address coping issues that may arise. Prepare documentation for the clinical record per Hospice standards in a timely manner. Participate in taking on-call responsibilities as necessary including weekends and evenings. Adhere to practice of confidentiality regarding patients, families, staff and the organization.
