In this role, you will be responsible for providing high-quality care to residents, ensuring their health needs are met with professionalism and empathy. Key Responsibilities: Resident Assessment & Care: Conduct comprehensive assessments of residents upon admission, throughout their stay, and when changes in condition occur. Monitor and evaluate residents' health status, identifying potential issues and implementing appropriate interventions. Care Plan Development: Develop, implement, and update individualized care plans for residents. Work with the healthcare team to ensure that care plans reflect the residents’ needs, preferences, and medical conditions. Medication Administration: Administer prescribed medications accurately and safely. Monitor for side effects and reactions, and document the administration in the residents' medical records. Clinical Procedures & Treatments: Provide direct nursing care, including wound care, catheter care, IV therapy, vital sign monitoring, and other necessary medical treatments. Follow infection control protocols and ensure a safe and clean environment for residents. Coordination of Care: Collaborate with physicians, other nurses, physical therapists, dietitians, and other healthcare professionals to coordinate care. Communicate effectively with families regarding residents' care, progress, and needs. Resident and Family Education: Educate residents and their families about healthcare needs, treatments, medication management, and lifestyle choices that support well-being and recovery. Supervision & Leadership: Supervise and mentor nursing assistants (CNAs) and other healthcare staff, ensuring they follow care protocols and maintain high standards of care. Provide training and guidance as necessary. Documentation & Compliance: Maintain accurate and thorough documentation of nursing care provided, including assessments, treatments, medications, and progress. Ensure compliance with all regulatory requirements and facility policies. Emergency Response & Critical Care: Respond quickly and effectively to emergency situations, providing necessary interventions, stabilization, and communication to physicians and emergency personnel. Qualifications: Registered Nurse (RN) with active licensure in New Mexico Minimum of 2 years of nursing experience, preferably in [skilled nursing, long-term care, hospital setting, etc.]. Strong knowledge of nursing practices, clinical procedures, and healthcare regulations. Excellent communication and interpersonal skills, with the ability to work well with residents, families, and the healthcare team. Ability to assess, prioritize, and manage multiple patient needs effectively. Compassionate, patient-centered approach to care with a strong focus on promoting the dignity and well-being of residents. Physical Requirements: Ability to lift, carry, and move patients, as necessary. Ability to stand or walk for extended periods. Ability to perform tasks that may require bending, reaching, or stretching. Ability to work in environments with exposure to infectious materials, cleaning chemicals, and physical demands typical of a healthcare setting. Key Responsibilities Leadership •Model clinical confidence and accountability across all levels of the organization •Identify clinical development opportunities for regional clinicians and facility personnel •Apply problem-solving and root cause analysis skills to resolve complex clinical concerns •Articulate and champion the clinical vision and standards of excellence • Support and facilitate Peaks Healthcare core values, practice standards, and code of conduct • Establish priorities to leverage resources proactively and avoid crisis situations Staff Training and Development •Identify facility strengths and clinical development needs; mentor and develop co-worker and facility clinical leadership in staffing, survey readiness, quality management, and customer service •Collaborate with interdisciplinary teams to identify educational needs and communicate them to appropriate departments •Participate in training programs to meet organizational needs and ensure training consistency Clinical Operations •Use standard company reports and tools to monitor contract labor, open positions, supply costs, PPD, and other indicators to analyze trends and meet budget expectations •Monitor staffing ratios to validate compliance with company, state, and federal requirements •Collaborate with Clinical Reimbursement to ensure documentation adequacy for medical necessity and reimbursement compliance •Train and direct leadership on survey process management; ensure facility team competency with survey management and implement corrective plans when weaknesses are identified •Review the Statement of Deficiencies (2567) to determine opportunities for Informal Dispute Resolution (IDR) and facilitate plan of correction completion to meet deadlines •Oversee Risk Management reporting, ensuring high-risk events and reportable occurrences are communicated and submitted in accordance with State and Federal requirements •Partner with the Clinical Director of Pharmacy Services on pharmacy initiatives; maintain current knowledge of regulations and communicate changes to the appropriate leaders Continuous Quality Improvement •Communicate regularly with the Leadership Team regarding facility and regional quality and operational concerns •Identify trends and collaborate to implement solutions; monitor effectiveness and sustain improvement •Validate facility site visits are completed, analyze results, conduct root cause analysis, and provide timely feedback with recommended changes •Utilize quality management reports (QIQM, Event Management, and other tools) to proactively evaluate and manage operational systems •Monitor and validate survey trends to manage resident care delivery and sustain substantial compliance Qualifications •Registered Nurse (RN) license required •5–8 years of clinical experience in Long Term Care/Skilled Nursing Facilities •Working knowledge of long-term care clinical and operational systems, including reimbursement regulations, nursing practices, and federal/state regulatory requirements •Strong analytical skills with the ability to understand financial systems and budgetary management •Ability to travel by car and plane, overnight as needed; frequent travel throughout the State of New Mexico expected Benefits •Competitive pay •401K •Health, dental, and vision insurance •Paid Time Off (PTO) •Supportive and collaborative work environment Grow With Us At Peaks Healthcare, we are committed to developing our team.
