Essential Duties and Job Responsibilities Patient Assessment and Care Planning: Conduct thorough assessments of residents' physical, cognitive, and emotional needs. Develop and implement individualized care plans in collaboration with the interdisciplinary team, including physicians, therapists, and other healthcare professionals. Regularly evaluate and update care plans based on residents' changing needs and responses to interventions. Medication Administration and Monitoring: Administer medications and treatments as prescribed by physicians, ensuring accuracy and safety. Monitor residents' responses to medications and treatments, documenting any adverse effects or changes in condition. Vital Signs and Health Monitoring: Monitor and document vital signs, such as temperature, pulse, respiration, and blood pressure. Observe residents for any signs of illness or changes in condition and promptly report any concerns to the physician. Wound Care and Other Specialized Care: Collaborate with wound care specialists or other healthcare professionals to ensure optimal wound healing. Supervision and Training: Supervise and provide guidance to licensed practical nurses (LPNs) and certified nursing assistants (CNAs). Provide orientation and training for new staff members. Communication and Documentation: Maintain accurate and complete documentation of residents' care, including assessments, interventions, and evaluations. Communicate effectively with residents, families, and other healthcare professionals. Participate in interdisciplinary team meetings to coordinate care and address resident needs. Quality Improvement: Participate in quality improvement initiatives to ensure the delivery of high-quality care. Stay current with best practices and evidence-based guidelines for nursing care in skilled nursing facilities. Must possess a nursing degree from an accredited college or university. Must possess a current, unencumbered, active license to practice as an RN in this state. BSN preferred. Must maintain and have an active CPR/BLS during employment. Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting. Experience in electronic medical records documentation. MDS Coordinator - Registered Nurse Status: Full-time Schedule: Days, Mon-Fri Wage: $110,000 - $131,000 DOE Location: Avamere Rehab of Cascade Park - 801 SE Park Crest Ave. Vancouver, WA 98683 Apply at www.TeamAvamere.com Job Summary The MDS Coordinator (Minimum Data Set Coordinator) is the primary point of contact for the review and coordination of patient care plans to promote quality care and services to residents. This role works with other nursing department colleagues to ensure that patient care is coordinated and communicated effectively with the clinical team. MDS job coordinators determine the health status, care plans, and procedures for intake of residents according to state and federal standards. The successful candidate will possess a strong understanding of healthcare regulations and have experience in clinical assessment and documentation. Essential Duties and Job Responsibilities Review resident care plans for appropriate goals, problems, approaches, and revisions based on nursing needs. Ensure all personnel involved in providing care to the residents are aware of care plans. Schedule and facilitate care plan conferences. Monitor and guide the completion of PPS and OBRA assessments. Ensure accuracy of clinical assessments and documentation. Ensure legal compliance with all state and federal guidelines, including, but not limited to following MDS processes. Manage the utilization review (UR) process. Oversee the MAR’s treatment, flow sheets, and physician’s orders. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident and resident’s response to care. Coordinate with other departments to ensure consistency in resident care. Participate in facility surveys by authorized government agencies. Report findings and make recommendations to management. Act in a supporting role to the Administrator and Director of Nursing of the facility. Nursing degree from an accredited college or university. In accordance with state and federal regulatory requirements, maintain a current, unencumbered, active license to practice as a registered nurse in this state. Oregon: RN is required Washington: RN is preferred MDS certification preferred. Knowledge and experience in the RAI process, CAAs, RUG categories, and MDS 3.0. Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care communities.
