In-Home Services CNA

The In Home Services CNA is responsible for providing routine care and assisting participants or clients with activities of daily living, restorative and supportive care. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Follows plan of care and assists participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, hand and toileting as assigned or needed. Monitors participants’ health status such as vital signs, weights, skin condition, appetite and changes in functional ability and reports pertinent information to clinical supervisor in a timely manner. Reports any variance, accident or unusual occurrence to the appropriate personnel. Attends in-service and continuing education programs as scheduled and necessary. Initiate the Plan of Care and makes necessary revisions as patient status and needs change. 4. Provide education to patients and family members on proper home health procedures and strategies. 5.
